Charge Entry Services for US Practices
Charge entry posts the coded visit into the billing system so a claim can be built. Patient, payer, codes, units, modifiers, provider, place of service, and fee. A charge that never gets entered never gets paid. Vector Medical Billing does this inside your EHR, from Jamaica, NY, for all 50 states.
What charge entry is in medical billing
In the revenue cycle, charge entry sits after coding and before claim scrubbing. It turns a coded encounter into billable lines in the practice management system.
If demographics are wrong, the claim rejects for patient data. If the payer is wrong, it routes to the wrong plan. If a modifier or unit is missing, the payer underpays or denies. If the visit never posts, there is no claim at all. That is why charge entry is not “just typing.”
Vector Medical Billing enters charges as part of the monthly billing engagement, then moves clean lines into scrubbing. Submission averages 24 hours after the charge is received. Coding detail is on the medical coding page. The full claim cycle is on medical billing services.
Charge capture, coding, and charge entry
Three different jobs. Mixing the names is how missed charges and wrong claims get blamed on “billing” in general.
Charge capture
Clinical. Someone records that a billable service happened — in the note, the order, or the superbill. If capture fails, entry has nothing to post.
Medical coding
Interpretive. ICD-10-CM, CPT, and HCPCS are assigned from the documentation. Entry uses those codes. It does not invent them from the schedule.
Charge entry
Data. The coded lines plus demographics, insurance, fee, provider, and place of service go into the system that builds the claim.
What charge entry puts on every line
Each field below is a denial or a missed dollar when it is wrong or blank.
| Field | What is entered | What fails if it is wrong |
|---|---|---|
| Patient demographics | Name, DOB, address, subscriber ID | Identity mismatch / missing information rejections |
| Insurance and plan | Payer, plan, financial class, COB order when needed | Wrong-payer routing and coverage denials |
| Date of service | The date the service was performed | Timely-filing and date mismatches |
| Procedure codes | CPT / HCPCS from the coded note | Wrong service billed, or no billable line at all |
| Diagnosis link | ICD-10-CM tied to the procedure that needs it | Medical-necessity and linkage denials |
| Units and modifiers | Dose, time units, 25 / 59 / 26 / TC when supported | Underpayment, bundling edits, unit denials |
| Provider | Rendering and billing NPI | Provider-mismatch rejections |
| Place of service | Office, outpatient, inpatient, ASC, and so on | POS-driven denials and wrong fee schedule |
| Charge amount | Practice fee schedule, not a guessed allowed amount | Books that cannot be reconciled to the remittance |
Work runs inside Athenahealth, eClinicalWorks, Tebra, AdvancedMD, DrChrono, NextGen, CareCloud, Practice Fusion, CureMD, Office Ally, and other systems confirmed before the account starts. The clinical workflow does not change.
Missed charges, reconciliation, and charge lag
These are the charge-entry problems practices search for when collections look low and the payer is not the real cause.
Missed charges
A visit or procedure was documented and never posted. There is no claim to deny. The dollar simply never entered the cycle. High-volume days, add-on procedures, and injections are common gaps.
Charge reconciliation
Posted charges are matched to the appointment schedule and the documented encounters. Unbilled visits stay on an exception list. That check is how missed charges are found before timely filing runs out.
Charge lag
The gap between the date of service and the day the charge posts. Long lag pushes submission later and raises days in A/R even when the payer pays on time. Same-day or next-day entry keeps that gap short.
How Vector Medical Billing runs charge entry
One pass from source to scrub-ready charge. Gaps become queries, not invented lines.
Take the source
Note, superbill, or EHR charge queue. When they disagree, the signed note wins.
Check patient and payer
Demographics and insurance are validated before procedure lines are built.
Post coded lines
CPT/HCPCS, diagnosis link, units, modifiers, provider, POS, and fee.
Reconcile, then release
Match the schedule. Release complete charges to scrubbing. Hold exceptions.
Charge entry on live claims is inside the monthly billing fee. The written rate is on the medical billing page. Ask for a written rate.
Charge entry questions
What is charge entry in medical billing?
Posting the coded encounter into the billing system with patient, insurance, codes, units, modifiers, provider, place of service, and fee so a claim can be created.
How is charge entry different from charge capture?
Capture records that a service happened. Entry puts the coded service and claim fields into the practice management system.
How do you catch missed charges?
By reconciling posted charges to the schedule and documented encounters, then working the exception list before timely filing expires.
What is charge lag?
The days between the visit and the posted charge. Shorter lag means the claim can leave sooner and days in A/R does not inflate before submission.
Do we change our EHR for charge entry?
No. Entry runs in the system you already use.
Is charge entry billed separately?
Not on the full monthly engagement. It sits with coding and submission. Details are on the medical billing fee section.
