Medical Coding Services for US Practices and Hospitals
Medical coding assigns ICD-10-CM, CPT, HCPCS, and ICD-10-PCS from the signed note — with modifiers, NCCI edits, and HCC capture when the contract needs risk scores. Vector Medical Billing’s AAPC and AHIMA certified coders work by specialty, inside your EHR, from Jamaica, NY, for all 50 states.
What medical coding is in the revenue cycle
Coding translates the signed note into standard codes. Billing submits those codes, posts the payment, and works the denial.
A code the note does not support is an audit problem. A code the note does support, left off the claim, is money the payer never sees. The coder reads the signed note, assigns diagnosis and procedure, checks modifiers against NCCI, and queries the provider when documentation is incomplete. The code is not guessed from the schedule.
Live coding sits inside the monthly billing engagement. Charge entry posts the coded lines. Scrubbing and denials stay on medical billing services and claims submission.
Medical coding vs medical billing
Two jobs. Mixing the names is how undercoding and unpaid claims get blamed on “billing” in general.
Medical coding
Interpretive. ICD-10-CM, CPT, HCPCS, ICD-10-PCS, modifiers, and HCC when needed — assigned from the documentation and queried when the note is thin.
Medical billing
Operational. Eligibility, charge posting, scrubbing, clearinghouse submission, ERA posting, denials, and A/R. Uses the codes coding produced.
Charge entry
Data. The coded lines plus demographics, insurance, fee, provider, and place of service go into the system that builds the claim.
Code sets medical coding services cover
The sets have to agree. Diagnosis supports procedure. Modifier survives the edit. Risk score reflects conditions actually managed.
| Code set | What it captures | Where accuracy matters |
|---|---|---|
| ICD-10-CM | Diagnosis and medical necessity | Specificity, laterality, encounter type, combination codes, CPT linkage |
| CPT / E/M | Procedures and evaluation & management | Level selection (MDM or time), add-ons, global package rules |
| HCPCS Level II | Drugs, J-codes, DME, supplies | Units, waste, NDC when the payer requires it |
| ICD-10-PCS | Inpatient facility procedures | MS-DRG grouping. Physicians still use CPT for professional claims |
| Modifiers & NCCI | Distinct, reduced, or professional/technical services | 25, 59 / X{EPSU}, 26 / TC, bundling edits, MUEs |
| HCC / risk adjustment | Chronic conditions for MA and value-based contracts | Documented, managed conditions only — no unsupported RAF |
Dental claims use CDT, not CPT. A medical plan pays a dental service only when that service meets a medical benefit. Coders work inside Athenahealth, eClinicalWorks, Tebra, AdvancedMD, DrChrono, NextGen, CareCloud, Practice Fusion, CureMD, Office Ally, or another system confirmed before start.
HCC risk adjustment and inpatient ICD-10-PCS
Two clusters competitors treat as separate products. Vector Medical Billing covers both when the account needs them.
HCC / risk adjustment coding
For Medicare Advantage and value-based contracts, chronic conditions that are documented and managed in the chart are captured for risk scores. A diagnosis is not added to raise RAF. Recapture follows what the note supports for that year — not a copy of last year’s problem list alone.
Inpatient facility ICD-10-PCS
Acute inpatient stays use ICD-10-CM and ICD-10-PCS for MS-DRG grouping on the facility claim. The physician’s professional claim for the same stay still uses CPT. Mixing those paths is how DRG and professional billing both go wrong.
Specialty medical coding
Cardiology, orthopedics, behavioral health, oncology, radiology, and the rest of the roster do not share one coding habit.
Coder matched to the specialty
The coder on the account works that specialty’s bundling rules, global periods, and payer quirks. Full list on medical billing specialties.
Provider query when the note is thin
If documentation cannot support the code, the coder asks for the missing fact. A diagnosis is not added to make the claim pay or to raise a risk score.
AAPC and AHIMA credentials
Coders hold AAPC and/or AHIMA certifications. Specialty assignment matters more than a generic credential alone.
How a chart is coded
The note is the source. The schedule is not.
Chart review
Open the signed note in your EHR. Read what was documented for that date of service.
Assign codes
ICD-10-CM, CPT or HCPCS, modifiers, HCC when needed — diagnosis supports procedure.
Provider query
If the note cannot support a code, ask for the missing fact before the claim is built.
QA release
Second look for level, modifier, and NCCI issues, then release to charge entry.
Physician, outpatient, and inpatient coding
Same method. Different code path and claim form.
Physician / pro-fee
Office, clinic, urgent care, hospitalist professional claims: ICD-10-CM with CPT/HCPCS on the CMS-1500 / 837P path.
Outpatient and ASC
Hospital outpatient and surgery center encounters: CPT, HCPCS, implants, multiple-procedure rules that feed APC payment.
Inpatient facility
Acute inpatient stays: ICD-10-CM and ICD-10-PCS for MS-DRG. The physician’s professional claim for the same stay still uses CPT.
Medical coding audit
A look back at charts already billed — undercoding, repeat denial reasons, fee-schedule gaps — is a separate engagement. It is not folded into the monthly percentage. See billing and coding audit.
How coding is priced
Coding on current claims is inside the full-cycle monthly fee. Coding-only vendors in the US often quote roughly $1–$5+ per chart; that is a market benchmark, not Vector’s published client rate. Full-cycle bands are on the medical billing page. Ask for a written rate.

Medical coding questions
Straight answers you can check against the sections above.
What is the difference between medical coding and medical billing?
Coding assigns diagnosis and procedure codes from the note. Billing submits the claim, posts payment, and works denials. They are sequential jobs, not synonyms.
What code sets do you use?
ICD-10-CM, CPT, HCPCS, ICD-10-PCS for inpatient facility procedures, modifiers against NCCI, and HCC when the contract is risk-adjusted. Dental uses CDT.
Do you do HCC risk adjustment coding?
Yes, when Medicare Advantage or value-based contracts need it. Only conditions documented and managed in the chart.
Do you code inpatient facility claims?
Yes, with ICD-10-CM and ICD-10-PCS for MS-DRG. Professional claims for the same stay still use CPT.
Which credentials do the coders hold?
AAPC and AHIMA certifications. Coders are matched to specialty.
What happens when the note is incomplete?
The coder queries the provider for the missing fact. Codes are not invented to force a claim through.
Do we change our EHR?
No. Coders log into the system you already use, confirmed before the account starts.
Is coding an extra fee?
Current claims: inside the monthly engagement. Retrospective coding audit: quoted separately.
