Multi-Location Practice Billing
Two offices under one group are two service locations. A claim that always prints the main address, or a report that only shows the group, hides the site that is actually losing cash. Vector Medical Billing, in Jamaica, New York, puts the address where the service happened on the claim and reads A/R by location.
What multi-location billing includes
One billing desk for every site. The facility address and the place of service still follow the visit.
The cycle matches medical billing at each site: eligibility, charges, coding, submission, posting, denials, and A/R. CMS-1500 box 32 is the location where the service was furnished. A visit in the second office does not inherit the flagship street address because that address was first in the practice management system. Some Medicaid programs enroll by site; a payer that has only the original office on file will not pay the new one until that enrollment exists. Credentialing for an added address is a separate engagement.
The fee is a percentage of net collections. The written rate is on pricing. Credentialing, a retrospective coding audit, and old A/R stay separate engagements.
What sits on this fee
- Service facility address taken from the site of the visit
- Place of service checked against that site, not a default
- A/R and denials readable by location
- New-site charges held where the payer has not enrolled that address
Where multi-location claims fail
The clinical note can be complete and the claim still names the wrong building.
Every claim shows the main office
Staff open the schedule at the second site and the billing file still carries the original address. Medicare and most commercial payers want the place the service was rendered. A copied address is a denial, and on a review it is a compliance problem.
Place of service copied from the first clinic
An office (11), an urgent care site (20), and a hospital outpatient department (22) do not pay the same way. A second location that is provider-based, or urgent care, cannot ride the office place of service of the original clinic.
The weak site disappears in the group total
Collections can look steady while one address carries the eligibility gaps and the aged balances. Reporting splits by location so that site is visible. See RCM reporting.
How a multi-site claim is built
Same group. The location fields are not optional.
| Check | How Vector handles it |
|---|---|
| Service address | The building where that encounter happened. |
| Place of service | Matches that building's setting, not the flagship clinic. |
| Enrollment | If a payer enrolls by site, claims for the new address wait until that site is active. |
| Labs and drugs | CLIA and drug inventory stay with the site that performed or supplied them, when those services are billed. |
| Report | Charges, denials, and A/R by location, plus the group total. |

Not inside the monthly percentage
The monthly cycle bills claims that are already in the system. These are scoped on their own.
Written rate
The fee is a percentage of net collections. No setup fee on the monthly cycle. Month to month. The written rate follows volume, payer mix, and how much of the cycle you hand over.
See pricing or request a written rate after the free audit.
Separate work
Enrolling a new street address with each payer, a site-by-site coding audit, and old A/R at a location you are closing are quoted separately.
Audit each address, not only the group rollup
Send the practice shape, monthly visit or case volume, top payers, and the EHR name. We reply with what the open A/R shows and a written rate.
Multi-location billing questions
Straight answers for this practice shape.
Do we need a separate NPI for every office?
Not always. Some groups bill a group NPI and put the service location in the facility address. Some payers, especially Medicaid, enroll the site itself. The audit checks which of those your payers require before claims drop for a new address.
Can both offices share one billing workflow?
Yes. The workflow is shared. The address and place of service on each claim are not.
What if one site is urgent care and one is an office?
They are different places of service and often different payer contracts. Use this page with urgent care billing.
How is the fee set?
A percentage of net collections. No setup fee. Month to month. The written rate follows volume and how many sites are in the cycle.
Is adding a location included?
Running claims for a location that is already enrolled is part of the monthly work. Payer enrollment for a new address is separate.
