General Surgery Billing Services
General surgery billing follows the global package — pre-op and post-op visits inside it are not extra E/M. Vector Medical Billing, in Jamaica, New York, scrubs 0/10/90-day clocks, modifier 57, assistants, and same-incision hernia rules inside the EHR you already use.
What general surgery billing services include
Full-cycle RCM for general surgeons — the operative note drives the claim, and the global clock decides which later visits stay inside the package.
The operating cycle matches medical billing services: eligibility and prior auth, charge capture from office and operative reports, ICD-10 / CPT coding, scrub, clearinghouse submission, payment posting, denial management, and A/R follow-up. The scrub watches 0-, 10-, and 90-day globals, modifiers 24 / 25 / 57 / 58 / 78 / 79, assistant and co-surgeon eligibility, multiple-procedure ranking, same-incision hernia and incidental appendectomy edits, place of service, and elective-case auth.
In scope: office and consult E/M, hernia repair (including current anterior abdominal families with 0-day globals versus inguinal and related 90-day codes), laparoscopic and open cholecystectomy, appendectomy when not incidental, bowel and soft-tissue procedures performed by the general surgeon, breast procedures when on this account, and related professional claims at ASC or hospital. Dedicated gastroenterology billing covers office endoscopy that is not a general-surgery operative claim. Orthopedics keeps fracture and joint globals. Facility technical fees stay on the center’s claim.
General surgery is priced as a percentage of net collections with other high-scrub surgical work. The written rate follows case mix and volume on pricing. Credentialing, a retrospective coding audit, MIPS, and out-of-network / IDR stay separate engagements.
What sits on this surgery fee
- Global clocks and decision-for-surgery visits tracked per case
- Assistants, multiples, and unbundled abdominal components scrubbed
- HIPAA workflows, a signed BAA, and PHI limited to staff on the account
Where general surgery claims fail
Patterns global-surgery rules and specialty surgical RCM keep naming after the OR.
Post-op billed outside the package
Routine related visits inside a 90-day global submitted as new E/M. Modifier 57 used on a minor (0- or 10-day) procedure. Modifier 24 used when the problem is the surgery itself.
Hub rule: pre-op and post-op inside the global are part of the surgical package.
Components unbundled from the abdomen
Exploratory laparotomy stacked on an open abdominal procedure. Incidental appendectomy of a normal appendix. Hernia repaired through the cholecystectomy incision billed as a second surgery.
The op note decides what was separate — not the charge master.
Assistant and multiple-procedure misses
Modifier 80 on a code that does not allow an assistant, or 80 used when the payer wants AS for a PA. Second procedure expected at 100% when multiple-surgery ranking pays 50% of the lower fee-schedule amount.
Role and ranking before the claim leaves — not after the underpay.
Pre-op and post-op inside the global period
Hub language made operational: the package includes routine related care — with narrow exceptions.
Medicare assigns 000 (0-day), 010 (10-day), or 090 (90-day) globals. Major surgery (90-day) includes the day before, the day of surgery, and 90 days after for routine related care by the surgeon or same-specialty group. Minor procedures generally do not pay a separate visit the same day unless a significant, separately identifiable E/M unrelated to the decision for that minor procedure is documented with modifier 25.
Modifier 57 identifies the E/M that results in the initial decision for major surgery on the day of or the day before the procedure. It is not for planned surgery already decided, and it is not for 0- or 10-day procedures. During the global, modifier 24 is for an unrelated E/M; 58 for a staged or more extensive related procedure; 78 for an unplanned return to the OR for a related complication; 79 for an unrelated procedure. Routine wound checks and typical post-op visits stay in the package.
Global checklist
- Look up the global indicator on the CPT actually billed
- 57 only for the first decision for a 90-day case
- 24 / 58 / 78 / 79 match the note — not a habit modifier
Assistants, co-surgeons, and multiple procedures
The second name in the room is payable only when the code and the note both allow it.
Assistant and co-surgeon
Physician assistants-at-surgery use 80, 81 (minimum), or 82 (teaching setting when a qualified resident is not available). Non-physician assistants often need AS. Medicare pays a reduced percentage and only when the assistant indicator and medical necessity support it. Co-surgeons (62) each document a distinct part of the procedure — a second surgeon who only retracts is not automatically a co-surgeon. Team surgery (66) is rare and payer-specific.
Multiple surgery ranking
When the same surgeon performs more than one procedure in a session and the codes carry a multiple-surgery indicator, Medicare ranks by fee-schedule amount: the highest typically pays 100% and subsequent eligible procedures 50%. Endoscopic families may use different pricing before that ranking. Do not add a “multiple” modifier Medicare does not require, and do not expect full fee on every line. Bilateral 50 is priced as one amount when ranked.
Abdominal procedures and hernia boundaries
Exploration and same-incision repair are inside the primary procedure.
What is not separate
Exploratory laparotomy is not reported with another open abdominal procedure. An incidental appendectomy of a normal appendix is not separate. Cholecystectomy is not added to a hepatectomy that already includes gallbladder removal. A hernia repaired at the incision site of another open or laparoscopic abdominal procedure is inherent to closing that incision.
Hernia globals differ
Anterior abdominal hernia repairs (epigastric, incisional, ventral, umbilical, spigelian, and related current families) commonly carry a 0-day global. Inguinal, femoral, and many other hernia repairs remain 90-day. Do not apply one post-op policy to every hernia CPT. A medically necessary repair at a different site from the primary incision may be separately reportable.
Cholecystectomy and auth
Laparoscopic cholecystectomy (and open conversion when documented) is a professional surgical claim with its own global. Intraoperative cholangiography is separately reportable only when performed and not bundled by the code or payer. Elective cases often need prior auth that matches the planned approach — eligibility is not authorization.
Office procedures versus the OR claim
Place of service and who owns the facility fee.
Professional claim
Office minor procedures use POS 11 with the correct 0- or 10-day code. ASC and hospital cases use facility place of service on the surgeon’s professional claim. Do not bill an office POS for an OR case, and do not put facility technical or implant device charges on the surgeon’s CMS-1500 unless a separate facility agreement says otherwise.
Op note before codes
Approach (open vs laparoscopic), findings, what was removed or repaired, and whether a second procedure was at a different site belong in the operative report. Queries go back when the charge sheet lists a hernia or appendectomy the narrative does not support as separate. Laterality and conversion to open are coded from that note.
General surgery code families this service works
Illustrative anchors — the operative note and global indicator decide the final claim.
| Work type | Example | What to check | Common failure if skipped |
|---|---|---|---|
| Decision for major surgery | E/M + 57 | 90-day; first decision | 57 on minor procedure |
| Unrelated E/M in global | E/M + 24 | Different problem | Routine post-op billed |
| Minor same-day visit | E/M + 25 | Separately identifiable | Habit-25 |
| Return / staged / unrelated OR | 58 / 78 / 79 | Matches the note | Wrong global modifier |
| Assistant | 80 / 81 / 82 / AS | Code allows assistant | 80 on non-eligible CPT |
| Co-surgeon | 62 | Each documents a part | Retraction billed as 62 |
| Lap cholecystectomy | 47562 family | Approach; cholangiogram | Open code for lap |
| Hernia | 49591+ / inguinal families | 0-day vs 90-day | Same-incision hernia added |
| Appendectomy | 44970 when indicated | Not incidental normal | Added to every lap |
| Multiple procedures | Ranked 100 / 50 | Fee schedule order | Full fee on every line |
This page does not invent a general surgery denial percentage. The free audit reads your global E/M, assistant modifiers, and unbundled abdominal lines on the sample you send.
What a free audit line looks like
Illustrative composite only — not a client result and not a promised recovery.
| Field | Example entry |
|---|---|
| Documented service | Laparoscopic cholecystectomy; small umbilical hernia closed through a port incision; PA assisted; clinic visit day 10 for expected wound check |
| Billed codes | 47562, umbilical hernia repair, 80 on assistant line for the PA, 99213 day 10 |
| Issue found | Same-incision hernia not separately reportable; PA needs AS not 80 on many Medicare claims; day-10 visit is routine global care |
| Recommended correction | Bill 47562 only for that session; assistant with AS if the code allows an assistant and the note describes the role; drop the routine post-op E/M |
| Related check | Auth matches laparoscopic approach; facility fee stays off the professional claim |
| Why it matters | Extra hernia and post-op lines deny or recoup — caught in scrub or flagged in the free audit |

How Vector runs general surgery billing
Global package first — then the same revenue cycle.
Assign surgery
Coder and biller who already work 90-day globals, modifier 57, and abdominal NCCI.
Reconcile and code
Charge entry and coding from the operative report — queries when a second procedure is not supported.
Post and report
835 posting, denial work, A/R aging, monthly RCM report by case and global leakage.
Not inside the general surgery percentage
Related work stays separate so the quote matches the job.
ASC / hospital facility fees
Technical facility and implant charges stay on the ASC or hospital NPI. We bill the professional service your surgeons perform.
Credentialing / audit
Credentialing and a retrospective coding audit of global and unbundling patterns are separate engagements when scoped.
Underpay / OON
Underpayment recovery and OON / IDR stay separate when the dispute path is not ordinary denial work.
How general surgery billing is priced
Percentage of dollars that land — not a flat fee per case that ignores global leakage.
Written rate
General surgery is priced as a percentage of net collections because globals, assistants, and multiple-procedure rules take more scrub and follow-up time. No setup fee on the monthly cycle. Month to month. The written rate follows case mix and how much of the cycle you hand over.
See pricing or request a written rate after the free audit.
What we need to start
EHR access, monthly case mix (hernia, biliary, bowel, office minor), open A/R if you are leaving another biller, and a sample of global or unbundling denials. Switching usually overlaps so claim submission does not stop. See EHR integration if access setup needs a separate quote.
Audit the surgery claims that bill post-op outside the package
Send monthly case volume, top procedures, EHR name, and a sample of global or assistant denials. We reply with what the open A/R shows, whether the monthly fee fits, and a written rate — without inventing a clean-claim percentage.
General surgery billing questions
Straight answers tied to the sections above.
What does general surgery billing include?
Office and consult E/M, professional surgical claims, global tracking, assistants when supported, scrubbing, submission, posting, denials, and A/R. Facility fees stay on the ASC or hospital claim.
How much do general surgery billing services cost?
The fee is a percentage of net collections. No setup fee on the monthly cycle. The written rate follows case mix and payer mix.
Are post-op visits extra E/M?
Routine related visits inside the global package are included. Modifier 57 is for the first decision for a major surgery. Modifier 24 is for an unrelated problem during the global when the note supports it.
Can a same-incision hernia be billed with cholecystectomy?
No. A hernia repaired through the incision for another abdominal procedure is not separately reportable. A repair at a different site may be when it is medically necessary and documented.
Which modifier does a PA assistant use?
Often AS when the payer recognizes a non-physician assistant. Modifier 80 is for a physician assistant surgeon. The code must allow an assistant, and the operative note must describe the role.
Do we have to change our EHR?
No. Billers work in the system you already use — see EHR integration. Compatibility is confirmed before you commit.
Where does Vector provide general surgery billing?
The office is 15717 85th St, Jamaica, NY 11414-2620. Work is remote for general surgery practices in all 50 states. See the specialties hub.
Is Vector Medical Billing HIPAA certified?
HHS does not certify billing companies. Vector signs a BAA, limits PHI to staff on the account, and follows HIPAA workflows. A HIPAA security risk analysis is a separate engagement when scoped.
