Geriatrics Billing Services
Geriatrics billing centers on chronic care management and the annual wellness visit — kept apart from a problem-oriented visit when the rules allow it — plus advance care planning, cognitive assessment, post-discharge TCM, and nursing facility days when those sit on the account. Vector Medical Billing, in Jamaica, New York, runs that Medicare-heavy cycle inside the EHR you already use.
What geriatrics billing services include
Full-cycle RCM for geriatric medicine clinics, senior-focused primary care panels, and groups that round nursing facilities — Medicare and Medicare Advantage first.
The operating cycle matches medical billing services: eligibility and AWV frequency checks, charge capture from office, home, and facility notes, ICD-10 / CPT / HCPCS coding, scrub, clearinghouse submission, payment posting, denial management, and A/R follow-up. The scrub watches CCM enrollment and monthly time, AWV vs problem-visit split, ACP with or without the wellness day, cognitive assessment elements, TCM clocks after discharge, and nursing facility place-of-service codes.
In scope: multi-morbidity office E/M, IPPE and AWV, CCM and complex CCM, principal care management when applicable, TCM, advance care planning, cognitive assessment and care planning (99483), nursing facility initial/subsequent/discharge families, and home or residence visits when billed. Dedicated hospitalist inpatient day codes sit on hospitalist billing when that is a separate account. Broader adult primary-care dual-visit rules also appear on primary care billing.
The fee is a percentage of net collections. The written rate is on pricing. Credentialing, a retrospective coding audit, MIPS, and out-of-network / IDR stay separate engagements.
What sits on this geriatrics fee
- CCM, AWV, and problem visits kept on the lines the rules allow
- Post-discharge and facility clocks watched before claims age out
- HIPAA workflows, a signed BAA, and PHI limited to staff on the account
Where geriatrics claims fail
Patterns CMS AWV/ACP guidance and specialty geriatric RCM vendors keep naming on Medicare-heavy panels.
AWV booked too early
Subsequent AWV (G0439) needs the Medicare frequency window after the prior AWV or IPPE — typically at least 11 full months after the month of the last wellness service. Scheduling a few weeks early is a benefit-maximum denial, not a documentation problem. Collapsing an AWV into a 99214 loses the preventive benefit and misstates the day.
Eligibility should show the last wellness date before the slot is booked.
CCM without the checklist
CCM needs two or more qualifying chronic conditions, consent, a care plan, an initiating visit when required, and calendar-month time. Practices often do the work and never bill — or bill without a time log. CCM is not a substitute for the office visit or the AWV.
Vector verifies consent, plan, and minutes before 99490 (or practitioner-time CCM codes) leave.
TCM / facility clocks missed
After discharge, interactive contact within two business days and the face-to-face visit within 7 or 14 days decide 99495 vs 99496. Nursing facility prolonged time uses Medicare G0317 with high-level NF codes when time selects the level — not a generic prolonged CPT habit.
Discharge alerts and NF time documentation are scrub inputs, not afterthoughts.
CCM, AWV, and the problem visit — kept apart
Hub language made operational: three different jobs, three different claim stories when the rules allow it.
Chronic care management is a monthly, mostly non-face-to-face service for patients with multiple chronic conditions — consent, care plan, and time on the calendar month. The Annual Wellness Visit is a Medicare preventive planning encounter (G0438 initial, G0439 subsequent; IPPE G0402 in the first Part B year). Neither is a problem-oriented office visit.
When a significant acute or chronic problem is managed during an AWV day, bill the office E/M with modifier 25 on the problem line. Cost-sharing may apply to that E/M even when the AWV has none. Do not invent a sick visit for trivial findings discovered during wellness screening.
Separation checklist
- CCM billed only when monthly checklist is complete — not as a visit code
- AWV on G-codes with required elements and frequency cleared
- Problem E/M-25 only when MDM stands alone from the wellness plan
AWV, advance care planning, and cognitive assessment
Optional elements that pay when time and documentation are real — and cost-sharing rules differ by day.
AWV elements
Health risk assessment, medication and functional review, cognitive screening as required for the code, personalized prevention plan. It is not a head-to-toe physical. Optional SDOH risk assessment (G0136) may pair with the AWV under current CMS rules with modifier 33 when cost-sharing waiver applies.
ACP 99497 / 99498
Face-to-face discussion of goals and advance directives (forms optional). First 30 minutes (minimum time thresholds apply) plus add-on units. With AWV same day, same provider, same claim, and modifier 33, deductible and coinsurance for ACP are typically waived. Outside the AWV, standard Part B cost-sharing usually applies; document clinical change for repeated ACP in a year.
Cognitive assessment 99483
Structured cognition-focused evaluation with functional status, caregiver assessment, safety, and a written care plan — typically a substantial timed service. Generally billed separately from AWV when both occur; use modifier 25 when payer policy requires separation from another E/M on the same day. Incomplete element lists fail audits.
Monthly CCM and post-discharge TCM
The work geriatric clinics already do between visits — billed when clocks and consent are on file.
CCM / complex CCM
99490 and add-ons for clinical-staff time; practitioner-time CCM codes when the physician or QHP personally provides the minutes. Complex CCM needs moderate-to-high MDM and higher time. One billing practitioner per patient per calendar month.
TCM 99495 / 99496
Interactive contact within 2 business days of discharge; medication reconciliation; face-to-face within 14 days (moderate) or 7 days (high). Miss the visit window and the code is gone. Only one practitioner bills TCM for a given discharge.
Multi-morbidity office E/M
Established visits often support higher MDM when several chronic conditions with exacerbation or high risk are addressed that day. Diagnosis specificity matters for medical necessity and, on MA panels, annual chronic capture — see internal medicine billing for that diagnosis depth.
Nursing facility and home visits
Place of service and time rules differ from the clinic superbill.
Nursing facility E/M
Initial care (99304–99306), subsequent (99307–99310), and discharge day management use the facility code family — not office codes with a nursing-home POS. Levels follow MDM or time. Medicare prolonged services for NF use G0317 with 99306 or 99310 when time selects the visit and thresholds are met; do not report G0317 with discharge codes or lower-level NF visits that CMS excludes.
Medical necessity and total time (when used) must be in the note.
Home / residence
Home or residence E/M codes apply when the service is furnished in those settings. Medicare prolonged home services use G0318 with the high-level home codes when time rules are met. G2211 visit-complexity add-on may apply with qualifying office or home E/M under current CMS relationship rules — not with AWV alone.
Wrong POS is a systematic geriatric denial when clinic codes are used for facility work.

Illustrative anchors — the note, setting, and Medicare policy decide the final claim.
| Work type | Example | What to check | Common failure if skipped |
|---|---|---|---|
| Office E/M | 99202–99215 | Problems addressed; MDM or time | AWV day billed as sick-only |
| Medicare wellness | G0402; G0438; G0439 | Elements; frequency window | Booked too early |
| Same-day problem | E/M + 25 | Significant MDM apart from AWV | Trivial finding; missing 25 |
| CCM | 99490 / 99487 families | Consent; plan; calendar-month time | Work done, never billed |
| TCM | 99495 / 99496 | 2-business-day contact; 7/14-day visit | Window missed |
| ACP | 99497 / 99498 ± 33 | Time; same-day AWV for waiver | 33 used without AWV |
| Cognitive care plan | 99483 | Required elements; care plan | Incomplete assessment billed |
| Nursing facility | 99304–99316; G0317 | POS; time thresholds for prolonged | Office code in SNF |
This page does not invent a geriatrics denial percentage. The free audit reads your AWV frequency misses, CCM gaps, and facility coding on the sample you send.
What a free audit line looks like
Illustrative composite only — not a client result and not a promised recovery.
| Field | Example entry |
|---|---|
| Documented service | Medicare patient due for subsequent AWV; 25 minutes ACP discussion same day; moderate MDM for CHF exacerbation with med change; enrolled CCM with RN minutes logged that calendar month |
| Billed codes | 99214 only; no G0439; ACP without 33; CCM not billed |
| Issue found | AWV work collapsed into sick visit; ACP cost-sharing waiver missed; CCM checklist complete but unbilled; problem visit should be 99214-25 beside G0439 when both supported |
| Recommended correction | G0439 + 99214-25 + 99497-33 (same day/provider/claim as AWV) + CCM line if minutes/consent/plan clear for the month |
| Related check | Confirm last AWV/IPPE month for frequency |
| Why it matters | Three payable geriatric services reduced to one undercoded office visit — caught in scrub or flagged in the free audit |

How Vector runs geriatrics billing
Wellness, care-management, and setting first — then the same revenue cycle.
Assign geriatrics
Coder and biller who already work AWV frequency, CCM/TCM, ACP-33, 99483, and NF codes.
Reconcile and code
Charge entry and coding after the CCM/AWV/problem split — queries when time or elements cannot be supported.
Not inside the geriatrics percentage
Related work stays separate so the quote matches the job.
Credentialing
Provider credentialing for geriatricians and NPPs across Medicare Advantage and facility panels is a separate project.
Coding audit
A retrospective billing and coding audit of AWV, CCM, and ACP patterns is scoped on its own when you want a period review.
Underpay / OON
Underpayment recovery and OON / IDR are separate when the dispute path is not ordinary denial work.
How geriatrics billing is priced
Percentage of dollars that land — not a flat fee per visit that ignores CCM and AWV leaks.
Written rate
The fee is a percentage of net collections. No setup fee on the monthly cycle. Month to month. The written rate follows visit volume, Medicare mix, and how much of the cycle you hand over.
See pricing or request a written rate after the free audit.
What we need to start
EHR access, monthly visit and CCM census, nursing facility volume if any, open A/R if you are leaving another biller, and a sample of recent AWV or CCM denials. Switching usually overlaps so claim submission does not stop. See EHR integration if access setup needs a separate quote.
Audit the geriatric claims that collapse CCM, AWV, and the problem visit
Send monthly visit volume, Medicare mix, EHR name, and a sample of AWV or CCM denials. We reply with what the open A/R shows, whether the monthly fee fits, and a written rate — without inventing a clean-claim percentage.
Geriatrics billing questions
Straight answers tied to the sections above.
What does geriatrics billing include?
CCM, AWV kept apart from problem visits when both are documented, ACP, cognitive assessment, TCM, nursing facility and home coding when in scope, scrubbing, submission, posting, denials, and A/R follow-up.
How much do geriatrics billing services cost?
The fee is a percentage of net collections. No setup fee on the monthly cycle. The written rate follows visit volume and Medicare mix.
How are CCM and AWV kept apart from a problem visit?
CCM is monthly care management — not an office visit. AWV uses Medicare G-codes. A significant problem the same day as an AWV is a separate E/M with modifier 25. Do not collapse the three into one CPT.
When is ACP cost-sharing waived?
When 99497/99498 is same day, same provider, same claim as an AWV with modifier 33. Outside the AWV, standard Part B cost-sharing usually applies.
What is CPT 99483?
Cognitive assessment and care planning with required structured elements and a written care plan. Generally separate from AWV when both are performed and documented.
Do we have to change our EHR?
No. Billers work in the system you already use — see EHR integration. Compatibility is confirmed before you commit.
Where does Vector provide geriatrics billing?
The office is 15717 85th St, Jamaica, NY 11414-2620. Work is remote for geriatric practices in all 50 states. See the specialties hub.
Is Vector Medical Billing HIPAA certified?
HHS does not certify billing companies. Vector signs a BAA, limits PHI to staff on the account, and follows HIPAA workflows. A HIPAA security risk analysis is a separate engagement when scoped.
