Pain Management Billing Services
Pain billing is the injection, the fluoroscopy already in the code, and the drug — after the authorization the plan requires. Vector Medical Billing, in Jamaica, New York, scrubs level, laterality, LCD frequency, and RFA prerequisites inside the EHR you already use.
What pain management billing services include
Full-cycle RCM for interventional pain clinics — injections by level and side, auth before the day, and the drug line that matches what was given.
The operating cycle matches medical billing services: eligibility and prior auth, charge capture from procedure notes, ICD-10 / CPT / HCPCS coding, scrub, clearinghouse submission, payment posting, denial management, and A/R follow-up. The scrub watches ESI approach and level caps, facet add-on sequencing, fluoroscopy bundling, bilateral 50 vs LT/RT by payer and place of service, RFA diagnostic-block prerequisites, LCD frequency windows, and drug units with JW or JZ when buy-and-bill applies.
In scope: office/outpatient E/M, interlaminar and transforaminal epidural steroid injections, facet joint injections and medial branch blocks, radiofrequency ablation of facet nerves, sacroiliac joint injections and related nerve work when billed, large-joint and trigger-point injections, peripheral and sympathetic blocks when performed, spinal cord stimulator trial and permanent implant professional claims when in scope, generator programming when owned, and injectable drug HCPCS. Dedicated neurology billing covers EMG/EEG-heavy neuro clinics — interventional spine work stays here when that is the practice focus. Facility technical fees stay on the ASC or hospital claim.
The fee is a percentage of net collections. The written rate is on pricing. Heavy ASC implant volume is scoped with a written rate that reflects the mix. Credentialing, a retrospective coding audit, MIPS, and out-of-network / IDR stay separate engagements.
What sits on this pain fee
- Injection, drug, and auth tracked on one workflow
- Level, laterality, and LCD frequency scrubbed before submission
- HIPAA workflows, a signed BAA, and PHI limited to staff on the account
Where pain management claims fail
Patterns specialty interventional RCM vendors keep naming on injection days.
Fluoro stacked on the injection
77003 billed with 62321 / 62323 or facet codes whose descriptors already include imaging guidance. Habit from older coding years that still triggers NCCI edits.
Hub rule: fluoroscopy is in the injection code — do not add it again.
Level / frequency LCD misses
More than two transforaminal levels the same session, third-level facet add-ons with wrong units, or sessions past the rolling 12-month regional cap. Diagnosis list that does not meet the LCD.
Count levels from the note against the MAC article — not the template.
Auth and RFA prerequisites
Injection or RFA performed without the prior auth the commercial plan requires. Facet RFA billed after one diagnostic medial branch block when policy demands two positive sessions with documented relief.
Authorization and block history before the room — not after the denial.
Authorization, then the injection, then the drug
Hub language made operational: plan rules decide whether the day can be billed.
Commercial and Medicare Advantage plans often require prior authorization for ESI, facet interventions, RFA, SI procedures, and spinal cord stimulator work. Traditional Medicare may not need auth for every code, but LCD medical-necessity and frequency rules still apply — and hospital outpatient prior-auth programs can cover selected facet and implant codes. We confirm the plan’s list before the procedure is scheduled when the practice routes auth through billing.
When the injectate is buy-and-bill, the HCPCS J-code, units, NDC, and JW (discarded) or JZ (no discard) modifiers must match what was administered and wasted. Auth for the procedure does not automatically authorize an off-formulary or step-therapy drug. Same-day E/M needs modifier 25 only when a significant, separately identifiable evaluation is documented beyond the procedure.
Pre-day checklist
- Auth number on file when the plan requires it
- Diagnosis and conservative-care language match LCD
- Drug units and JW/JZ ready when buy-and-bill
Epidural steroid injections
Approach, region, and level — imaging already in the CPT.
Interlaminar and caudal
Cervical/thoracic interlaminar with imaging (62321) and lumbar/sacral interlaminar or caudal with imaging (62323) are not bilateral procedures under Medicare ESI articles — do not append 50. Imaging guidance is included. Retain needle-position and contrast documentation when the LCD expects it. Session and regional frequency limits apply on a rolling calendar, not calendar year alone.
Transforaminal ESI
64479 / +64480 (cervical/thoracic) and 64483 / +64484 (lumbar/sacral) are first level plus each additional level. Medicare typically allows two total levels per session for these families. Bilateral uses modifier 50 on professional claims when the code allows it; ASC facility lines often use separate RT/LT instead. Diagnostic selective nerve root blocks may need KX when the MAC distinguishes them from therapeutic ESI. Do not bill 77003 alongside these codes.
Facet injections, medial branch blocks, and RFA
Diagnostic intent in the note — two positives before ablation when policy requires it.
Facet / MBB levels
64490–64492 (cervical/thoracic) and 64493–64495 (lumbar/sacral) cover intra-articular facet injection and medial branch block by first, second, and third-or-additional level. Imaging is included. Bilateral at the same level uses 50 (or LT/RT per payer and site). Do not bill two primary codes for one level. Note must state diagnostic vs therapeutic intent.
RFA prerequisites
64633 / +64634 and 64635 / +64636 are reported per joint denervated, not per individual medial branch nerve. Most policies require two separate positive diagnostic MBBs with documented relief at the stated threshold before initial RFA. Repeat RFA has its own duration-of-relief and sessions-per-region rules. Hold the claim when the chart cannot show the second positive block.
Frequency and hospital OPD
Facet diagnostic sessions and RFA sessions per spinal region are capped under LCD and often under commercial medical policy. Selected hospital outpatient facet and RFA codes may also sit on CMS prior-authorization lists. Track region and rolling 12 months per patient — not only “last injection date” in the EHR template.
SI joint, implants, and related lines
Correct professional code — facility code stays on the center.
SI joint and large joints
Physician SI joint injection with imaging is typically 27096; ASC facility may use G0260. SI and genicular nerve block families (e.g. 64451 / 64454) and SI RFA (64625) follow their own LCDs — do not interchange with lumbar facet codes. Large-joint injection 20610 / 20611 (with ultrasound) needs laterality and the drug line when applicable. Trigger points use 20552 (1–2 muscles) or 20553 (3 or more) by muscle count, not needle stick count.
SCS trial and permanent
Percutaneous electrode placement (63650) for trial and permanent generator implant (63685) are separate encounters with prior auth on most plans. Programming and analysis codes bill when the practice performs them. Global period on the implant means related post-op visits are included; staged trial-to-implant uses the correct staged/related modifier when documentation supports it. Facility device and implant charges stay on the ASC or hospital claim.
Pain code families this service works
Illustrative anchors — the procedure note, LCD, and auth decide the final claim.
| Work type | Example | What to check | Common failure if skipped |
|---|---|---|---|
| Office E/M | 99202–99215 ± 25 | Separate from injection | Habit-25 on procedure-only day |
| Interlaminar ESI | 62321 / 62323 | Imaging included; not bilateral | 77003 stacked; 50 appended |
| Transforaminal ESI | 64483 / +64484 | Level cap; bilateral rule | Third level same session |
| Facet / MBB | 64490–64495 | Level add-ons; intent | Wrong add-on units |
| Facet RFA | 64633–64636 | Two positive MBBs; per joint | One block on file |
| SI injection | 27096 / G0260 | Pro vs ASC facility | Wrong place-of-service code |
| Trigger point | 20552 / 20553 | Muscle count | Billed per stick |
| SCS | 63650 / 63685 | Trial vs permanent; auth | Auth missing; global ignored |
| Drug | J-code + JW/JZ | Units = given + waste | Units or NDC mismatch |
| Fluoroscopy | 77003 | Usually bundled | Separate line with ESI/facet |
This page does not invent a pain denial percentage. The free audit reads your fluoro bundling, level caps, auth gaps, and RFA prerequisites on the sample you send.
What a free audit line looks like
Illustrative composite only — not a client result and not a promised recovery.
| Field | Example entry |
|---|---|
| Documented service | Lumbar transforaminal ESI at L4–L5 and L5–S1 bilaterally under fluoroscopy; methylprednisolone buy-and-bill; commercial plan with PA |
| Billed codes | 64483-50 ×1, 64484-50 ×1, 77003, J1020 without JW/JZ; no auth number |
| Issue found | Fluoro unbundled; bilateral reporting may need LT/RT on ASC facility side; drug modifier missing; auth not on claim |
| Recommended correction | Drop 77003; report TFESI first and add-on levels with bilateral method the payer and site require; add JW or JZ and units; attach PA when required before resubmit |
| Related check | Rolling session count for lumbar region under LCD / plan medical policy |
| Why it matters | Highest-dollar injection day denied for bundling and auth — caught in scrub or flagged in the free audit |

How Vector runs pain management billing
Auth and level rules first — then the same revenue cycle.
Assign pain
Coder and biller who already work ESI level caps, facet add-ons, RFA prerequisites, and drug units.
Reconcile and code
Charge entry and coding from the procedure note — queries when level or side cannot be supported.
Post and report
835 posting, denial work, A/R aging, monthly RCM report by injection vs implant vs E/M.
Not inside the pain percentage
Related work stays separate so the quote matches the job.
ASC / hospital facility fees
Technical facility and implant device charges stay on the ASC or hospital NPI. We bill the professional service your physicians perform.
Credentialing / audit
Credentialing and a retrospective coding audit of ESI and RFA coding are separate engagements when scoped.
Underpay / OON
Underpayment recovery and OON / IDR stay separate when the dispute path is not ordinary denial work.
How pain management billing is priced
Percentage of dollars that land — not a flat fee per injection that ignores auth denials.
Written rate
The fee is a percentage of net collections. No setup fee on the monthly cycle. Month to month. The written rate follows injection versus implant mix and how much of the cycle you hand over.
See pricing or request a written rate after the free audit.
What we need to start
EHR access, monthly injection and implant census, top payers, open A/R if you are leaving another biller, and a sample of recent ESI, facet, or RFA denials. Switching usually overlaps so claim submission does not stop. See EHR integration if access setup needs a separate quote.
Audit the pain claims that stack fluoro or skip auth
Send monthly injection volume, top procedures, EHR name, and a sample of bundling, level, or prior-auth denials. We reply with what the open A/R shows, whether the monthly fee fits, and a written rate — without inventing a clean-claim percentage.
Pain management billing questions
Straight answers tied to the sections above.
What does pain management billing include?
Office E/M, ESI and facet work by level and side, SI injections, RFA when diagnostic blocks support it, SCS trial/implant professional claims when in scope, drug lines, scrubbing, submission, posting, denials, and A/R.
How much do pain management billing services cost?
The fee is a percentage of net collections. No setup fee on the monthly cycle. The written rate follows injection versus implant mix and payer mix.
Can 77003 be billed with an ESI or facet injection?
Usually no. Current ESI and facet descriptors include imaging guidance. Separate fluoroscopy is typically bundled.
How many medial branch blocks before RFA?
Most Medicare LCDs and commercial policies require two separate positive diagnostic MBBs with documented relief at the policy threshold before initial facet RFA is covered.
Do you handle prior authorization?
Yes when auth is in the billing scope for the account. We confirm plan requirements before high-dollar injection and implant days when the practice routes that work to us.
Do we have to change our EHR?
No. Billers work in the system you already use — see EHR integration. Compatibility is confirmed before you commit.
Where does Vector provide pain management billing?
The office is 15717 85th St, Jamaica, NY 11414-2620. Work is remote for interventional pain practices in all 50 states. See the specialties hub.
Is Vector Medical Billing HIPAA certified?
HHS does not certify billing companies. Vector signs a BAA, limits PHI to staff on the account, and follows HIPAA workflows. A HIPAA security risk analysis is a separate engagement when scoped.
