Psychiatry Billing Services
Psychiatry billing is an evaluation and management visit with a psychotherapy add-on when both are documented — plus medication management. Vector Medical Billing, in Jamaica, New York, keeps the med-check clock separate from the therapy minutes inside the EHR you already use.
What psychiatry billing services include
Full-cycle RCM for psychiatrists and prescribing APRNs — not a therapy-only queue with a different label.
The operating cycle matches medical billing services: eligibility and carve-out routing, charge capture from the note, ICD-10 / CPT coding, scrub, clearinghouse submission, payment posting, denial management, and A/R follow-up. The scrub watches E/M level versus documented medical decision making, psychotherapy add-on minutes, interactive complexity, telehealth place-of-service and modifiers under that payer’s rules, and prior auth on session blocks when the plan requires it.
In scope: psychiatric diagnostic evaluation with and without medical services, outpatient medication-management E/M, same-day psychotherapy add-ons when both are documented, interactive complexity when criteria are met, inpatient and consult psychiatry professional claims when the group bills them, and telepsychiatry under the payer’s current POS and modifier rules. Timed psychotherapy billed alone by therapists stays on the mental health billing page. Program-based substance-use, ABA, and peer-support claims are out of scope here unless separately engaged.
Psychiatry is priced as a percentage of net collections with other behavioral-health work. The written rate follows how much of the schedule is med management versus combined E/M-plus-therapy — see pricing. Credentialing, a retrospective coding audit, MIPS, and out-of-network / IDR stay separate engagements.
What sits on this psychiatry fee
- Med-management E/M kept separate from psychotherapy minutes
- Add-on codes scrubbed against the note before submission
- HIPAA workflows, a signed BAA, and PHI limited to staff on the account
Where psychiatry claims fail
Patterns payer psychotherapy-with-E/M policies and CMS psychiatry coding articles keep naming.
One blended note, two clocks
A single narrative billed as a high-level E/M and a long standalone psychotherapy code. Or 90832 / 90834 / 90837 stacked with an office visit by the same provider on the same day.
Hub rule: E/M plus a psychotherapy add-on — only when both are documented.
Wrong minutes or wrong level
E/M level chosen by total visit time while psychotherapy is also billed. Psychotherapy minutes that include the med-check. Add-on billed for fewer than 16 minutes of therapy. 90785 added because the visit “felt complex.”
MDM picks the E/M. Therapy minutes pick the add-on.
Med management as therapy
A refill and side-effect check coded as 90837. Diagnostic evaluation repeated every few weeks without a change in status the contractor allows. Therapist NPI on an E/M-plus-add-on claim.
Medication management is an E/M service for a qualified medical provider.
E/M with a psychotherapy add-on — when both are documented
Hub language made operational: two significant services, two clocks, one claim.
When the same psychiatrist, NP, or PA performs a separately identifiable evaluation and management service and psychotherapy on the same day, bill the office or outpatient E/M (for example 99213–99215) plus the psychotherapy add-on that matches therapy minutes: +90833 for 16–37 minutes, +90836 for 38–52 minutes, or +90838 for 53 or more minutes. Do not bill 90832, 90834, or 90837 with that E/M. Select the E/M level by medical decision making — problems addressed, data, and risk — not by adding therapy time into the visit clock. Psychotherapy time must be distinct; minutes spent on history, exam, and medication decisions do not count toward the add-on.
Some commercial plans automatically scrutinize or deny high-level E/M codes (such as 99214 / 99215) paired with a psychotherapy add-on. That is a payer edit, not a reason to invent a national denial percentage. The free audit reads whether your notes support both services without overlapping time. Prolonged-service codes are not stacked on top of E/M-plus-psychotherapy add-on claims.
Same-day checklist
- Separate E/M narrative supporting MDM
- Separate psychotherapy content and minutes
- Add-on code — not a standalone 9083x with the E/M
Medication management and diagnostic evaluation
A med check is not psychotherapy. An intake is not every follow-up.
Medication management
Medication review, side-effect monitoring, and prescription changes are evaluation and management services. Bill the E/M level the note supports. Do not use a psychotherapy code because the appointment was “psychiatry.” If the same visit also includes a separately documented psychotherapy service that meets the minute ranges, add +90833, +90836, or +90838 — otherwise stop at the E/M.
90791 and 90792
90791 is a psychiatric diagnostic evaluation without medical services. 90792 includes medical services and is the usual intake when a psychiatrist or other qualified medical provider performs the evaluation. These are not routine weekly follow-ups. Contractors typically allow a new diagnostic evaluation at the start of an illness, after a meaningful break in treatment, on inpatient admission for a psychiatric illness, or when mental status changes enough to require a fresh assessment — not on every return visit.
Interactive complexity and related boundaries
90785 is a defined communication factor — not a synonym for “difficult visit.”
When 90785 applies
Interactive complexity may be reported with psychiatric procedures when documentation shows a qualifying communication difficulty — for example managing maladaptive communication among participants, caregiver emotions that interfere with the plan, a sentinel event with mandated reporting, or use of an interpreter or play equipment to overcome a significant language or communication barrier. It is an add-on and is never billed alone.
When 90785 does not
Do not report 90785 with psychotherapy-for-crisis codes, or with an E/M service when no psychotherapy is also reported. A busy med-check without a qualifying communication factor is not interactive complexity.
Who bills the add-on
E/M with psychotherapy add-ons are for qualified medical providers — psychiatrists and other physicians, NPs, and PAs within scope and enrollment. A therapist’s standalone timed psychotherapy belongs on the mental health path under that clinician’s enrolled NPI, not under the psychiatrist as a collections shortcut.
Psychiatry code families this service works
Illustrative anchors — the note decides the claim.
| Work type | Example | What to check | Common failure if skipped |
|---|---|---|---|
| Diagnostic eval | 90791 / 90792 | New assessment vs follow-up | 90792 every visit |
| Medication management | Office / outpatient E/M | MDM in the note | Coded as 90837 |
| E/M + therapy 16–37 min | E/M + 90833 | Separate clocks | 90832 with E/M |
| E/M + therapy 38–52 min | E/M + 90836 | Therapy minutes only | Med time in the add-on |
| E/M + therapy 53+ min | E/M + 90838 | Documented content | Blended high E/M + 90837 |
| Interactive complexity | +90785 | Qualifying factor named | Habit add-on |
| Therapy alone | 90832 / 90834 / 90837 | No same-day E/M by that provider | Stacked with office visit |
| Telepsychiatry | POS + payer modifiers | That plan’s current rules | Wrong POS / missing modifier |
This page does not invent a psychiatry denial percentage. The free audit reads your E/M-plus-add-on pairs and med-management coding on the sample you send.
What a free audit line looks like
Illustrative composite only — not a client result and not a promised recovery.
| Field | Example entry |
|---|---|
| Documented service | Psychiatrist adjusts two psychotropic medications (moderate MDM), then documents 40 minutes of insight-oriented psychotherapy with start and stop times; services do not overlap |
| Billed codes | 99214 and 90837 |
| Issue found | Standalone psychotherapy used with E/M; should be the add-on family |
| Recommended correction | Bill 99214 + 90836; keep separate E/M and psychotherapy documentation; do not add prolonged services |
| Related check | If a commercial payer auto-denies 99214 + add-on, appeal with the two clocks — do not drop the E/M without reading the note |
| Why it matters | Wrong psychotherapy family with E/M denies or recoups — caught in scrub or flagged in the free audit |

How Vector runs psychiatry billing
Two clocks first — then the same revenue cycle.
Assign psychiatry RCM
Coder and biller who already split med-management E/M from psychotherapy add-ons.
Reconcile and code
Charge entry and coding from the note — queries when minutes or MDM are unclear.
Post and report
835 posting, denial work, A/R aging, monthly RCM report by med management versus combined visits.
Not inside the psychiatry percentage
Related work stays separate so the quote matches the job.
Facility psych unit fees
Hospital facility charges for inpatient psychiatry stay on the facility claim. We bill the professional psychiatry services your group owns.
Credentialing / audit
Credentialing and a retrospective coding audit of E/M-plus-add-on pairs are separate engagements when scoped.
Underpay / OON
Underpayment recovery and OON / IDR stay separate when the dispute path is not ordinary denial work.
How psychiatry billing is priced
Percentage of dollars that land — not a flat fee per med check that ignores add-on denials.
Written rate
Psychiatry is priced as a percentage of net collections because timed add-ons, E/M levels, and behavioral-health payer edits take more scrub and follow-up time. No setup fee on the monthly cycle. Month to month. The written rate follows visit mix and how much of the cycle you hand over.
See pricing or request a written rate after the free audit.
What we need to start
EHR access, monthly visit mix (med management versus combined), open A/R if you are leaving another biller, and a sample of add-on or high-level E/M denials. Switching usually overlaps so claim submission does not stop. See EHR integration if access setup needs a separate quote.
Audit the visits that bill a high E/M and a long therapy code together
Send monthly psychiatry volume, how often visits combine med management with psychotherapy, EHR name, and a sample of add-on denials. We reply with what the open A/R shows, whether the monthly fee fits, and a written rate — without inventing a clean-claim percentage.
Psychiatry billing questions
Straight answers tied to the sections above.
What does psychiatry billing include?
Diagnostic evaluations, medication-management E/M, same-day psychotherapy add-ons when both are documented, interactive complexity when criteria are met, scrubbing, submission, posting, denials, and A/R.
How much do psychiatry billing services cost?
The fee is a percentage of net collections. No setup fee on the monthly cycle. The written rate follows visit mix and payer mix.
When can you bill E/M and psychotherapy on the same day?
When both are separately documented. Use E/M plus +90833, +90836, or +90838. Do not bill 90832, 90834, or 90837 with that E/M. Select E/M by MDM; count only psychotherapy minutes toward the add-on.
Is medication management billed as psychotherapy?
No. Medication management is an evaluation and management service. Add psychotherapy only when a separately documented therapy service meets the add-on minute ranges.
Do we have to change our EHR?
No. Billers work in the system you already use — see EHR integration. Compatibility is confirmed before you commit.
Where does Vector provide psychiatry billing?
The office is 15717 85th St, Jamaica, NY 11414-2620. Work is remote for psychiatry practices in all 50 states. See the specialties hub.
Is Vector Medical Billing HIPAA certified?
HHS does not certify billing companies. Vector signs a BAA, limits PHI to staff on the account, and follows HIPAA workflows. A HIPAA security risk analysis is a separate engagement when scoped.
