Urology Billing Services
Urology billing covers office procedures and the surgical claim when the case leaves the office — cystoscopy bundling, the 2026 prostate biopsy family, stone treatment, and intravesical drugs. Vector Medical Billing, in Jamaica, New York, routes place of service and global periods inside the EHR you already use.
What urology billing services include
Full-cycle RCM for office-based urology and the professional surgical claim when the case moves to ASC or hospital.
The operating cycle matches medical billing services: eligibility and prior auth, charge capture from office notes and operative reports, ICD-10 / CPT / HCPCS coding, scrub, clearinghouse submission, payment posting, denial management, and A/R follow-up. The scrub watches cystoscopy NCCI bundling, place of service, laterality, global periods, the current prostate biopsy code family, stone-procedure sequencing, and intravesical drug units with JW/JZ when buy-and-bill.
In scope: office/outpatient E/M, diagnostic and therapeutic cystoscopy families, urodynamics, prostate biopsy under 55707–55715, BPH procedures (TURP, laser, UroLift, water vapor, and related), ESWL and ureteroscopic stone treatment, stent placement and removal when separately payable, vasectomy, intravesical instillation with drug J-codes (e.g. BCG), and catheter or related office procedures when documented. Facility technical charges belong to the ASC or hospital — the urologist’s professional claim stays on this account. Dedicated nephrology billing covers dialysis and monthly ESRD codes on a separate account when that is not urology work.
The fee is a percentage of net collections. The written rate is on pricing. Credentialing, a retrospective coding audit, MIPS, and out-of-network / IDR stay separate engagements.
What sits on this urology fee
- Office procedure claims and ASC/hospital professional claims on one workflow
- Cystoscopy bundling and global-period visits scrubbed before submission
- HIPAA workflows, a signed BAA, and PHI limited to staff on the account
Where urology claims fail
Patterns specialty urology RCM vendors keep naming on scope and stone days.
Diagnostic cysto unbundled
52000 billed with biopsy, fulguration, TURBT, or stent codes the same session. Diagnostic look is already in the therapeutic code. Lesion size or count in the note does not match the code selected.
Bill the most comprehensive cystoscopic service that the operative note supports.
Prostate biopsy on deleted 55700
Claims still using 55700 for 2026 dates of service deny outright. Notes that only say “prostate biopsy” without approach and imaging modality cannot pick among 55707–55715.
Imaging guidance is built into the new family — do not unbundle a separate US code for the same guidance.
Global / POS mix-ups
Routine post-op visits billed inside a 90-day global, or office POS used for an ASC case. ESWL and ureteroscopy laterality missing. Facility technical lines duplicated on the professional claim.
Place of service and global clock are set when the case is scheduled — not at denial.
Office procedures and the surgical claim when the case leaves
Hub language made operational: two settings, one account, correct place of service.
In-office work (POS 11) includes E/M, diagnostic cystoscopy when it stands alone, urodynamics, vasectomy when performed in clinic, catheter changes, and many intravesical instillations. Same-day E/M with a procedure needs modifier 25 only when a significant, separately identifiable evaluation is documented beyond the procedure itself.
When the case moves to ASC or hospital outpatient/inpatient, the professional claim uses the correct facility place of service. TURP, major stone procedures, and many prostate surgeries carry 90-day globals — routine related follow-up is included. Decision-for-surgery visits may use modifier 57 when documentation supports a true E/M that decides the major procedure. Facility fees and implants billed by the center are not duplicated on the physician claim.
Setting checklist
- POS matches where the service was performed
- Global clock started for major procedures; related E/M not double-billed
- Professional vs facility lines owned by the correct NPI
Cystoscopy bundling and office endoscopy
Highest-volume urology family — and the most common NCCI denial.
Diagnostic vs therapeutic
52000 is the look-only separate-procedure code. Biopsy (52204), fulguration and lesion treatment by size (52224 / 52234–52240 families), stricture dilation (52281), stent insertion (52332), and related therapeutic codes include the diagnostic look. Do not stack 52000 on the same session. Match lesion size and count in the note to the code — not the laser brand name.
Urodynamics
Bill by components actually performed (cystometrogram, uroflow, EMG, voiding pressure studies in the 51726–51741 range as documented). Not bundled into diagnostic cystoscopy by default. Same-day E/M still needs 25 only when separately identifiable. Payer medical-necessity policies for LUTS workups apply.
Prostate biopsy and BPH procedures
2026 biopsy family, TURP globals, and device-based BPH codes.
Biopsy 55707–55715
Select by approach (transrectal vs transperineal), imaging (US, MRI-fusion, in-bore), and systematic vs targeted. Add-on codes cover additional targeted lesions when documented. Do not submit deleted 55700 for 2026 dates of service.
TURP and laser
52601 is comprehensive: diagnostic cystoscopy, meatotomy, and urethral calibration/dilation the same session are typically bundled. Laser enucleation and related codes follow their own descriptors — do not unbundle components already in the primary code. Watch the 90-day global.
UroLift / Rezum / Aquablation
UroLift uses implant count codes with MUE caps. Water vapor thermotherapy and Aquablation (Category I as of 2026) are distinct mechanisms — do not interchange codes. Commercial plans often require prior auth even when traditional Medicare does not.
Stones, stents, and laterality
ESWL, ureteroscopy, and staged stent work — each with its own bundling map.
ESWL and URS
Extracorporeal shock wave lithotripsy (50590) and ureteroscopy with lithotripsy or basket extraction (e.g. 52353 / 52356 families) need laterality (RT / LT / 50). Imaging guidance is often built into current descriptors — verify before adding a separate guidance code. Staged ESWL and URS on different dates are distinct sessions; same-session combinations follow NCCI.
Stents and globals
Indwelling ureteral stent insertion (52332) and removal/exchange codes are checked against the primary stone or cystoscopy code for the same session. Many major urologic procedures carry 90-day globals — related uncomplicated follow-up is included. Unrelated problems in the global window need the correct modifier and diagnosis.
Intravesical drugs and office injectables
Instillation procedure and the drug — apart from the visit.
BCG and chemo instillation
Instillation administration (e.g. 51720) plus the drug J-code (BCG and other agents as supplied). Units, NDC, and JW/JZ on single-dose vials reconcile before submission. Do not collapse the drug into the E/M.
Auth and site of care
High-cost intravesical agents and some BPH devices need prior auth. Confirm medical vs pharmacy benefit before opening a vial from office stock.
Same-day visit
Modifier 25 only when a significant, separately identifiable evaluation is documented beyond the instillation. A planned BCG-only return typically does not support 25.
Urology code families this service works
Illustrative anchors — the operative note, POS, and payer policy decide the final claim.
| Work type | Example | What to check | Common failure if skipped |
|---|---|---|---|
| Office E/M | 99202–99215 ± 25 / 57 | Separate from procedure; decision for surgery | Habit-25 on procedure-only day |
| Diagnostic cysto | 52000 | Standalone only | Unbundled with therapeutic |
| Therapeutic cysto | 52204 / 52224 / 52234–52240 / 52332 | Lesion size; includes look | Wrong size tier |
| Prostate biopsy | 55707–55715 | Approach; imaging; systematic vs targeted | Deleted 55700 used |
| TURP / BPH | 52601 / 52441–52442 / 53854 / 52597 | Global; implant MUEs; auth | Components unbundled |
| Stone | 50590 / 52353 / 52356 | Laterality; session bundling | Missing RT/LT |
| Intravesical | 51720 + J-codes | Units; JW/JZ; separate from visit | Drug omitted |
| Urodynamics | 51726–51741 | Components performed | Panel assumed |
| Vasectomy | 55250 | 90-day global; 57 if decision visit | Routine post-op billed |
This page does not invent a urology denial percentage. The free audit reads your cystoscopy bundling, biopsy code selection, and global-period visits on the sample you send.
What a free audit line looks like
Illustrative composite only — not a client result and not a promised recovery.
| Field | Example entry |
|---|---|
| Documented service | Office cystoscopy with bladder biopsy of a 0.8 cm lesion; separately identifiable hematuria workup E/M same day |
| Billed codes | 52000 + 52204 + 99214-25 |
| Issue found | Diagnostic 52000 unbundled with biopsy; biopsy code may also be wrong size tier vs note |
| Recommended correction | Drop 52000; bill therapeutic cystoscopy with biopsy matching lesion documentation; keep 99214-25 only if E/M stands alone |
| Related check | Pathology of the specimen billed by the performing lab — not invented on the urology claim |
| Why it matters | NCCI denial or underpayment on the highest-volume office procedure — caught in scrub or flagged in the free audit |

How Vector runs urology billing
Bundling and place of service first — then the same revenue cycle.
Assign urology
Coder and biller who already work cystoscopy NCCI, 2026 biopsy codes, and surgical globals.
Reconcile and code
Charge entry and coding from office and operative notes — queries when lesion size or imaging modality cannot be supported.
Not inside the urology percentage
Related work stays separate so the quote matches the job.
ASC / hospital facility fees
Technical facility charges, implants billed by the center, and hospital outpatient packaging stay on the facility claim. We bill the professional service your NPI performs.
Credentialing / audit
Credentialing and a retrospective coding audit of cystoscopy and biopsy coding are separate engagements when scoped.
Underpay / OON
Underpayment recovery and OON / IDR stay separate when the dispute path is not ordinary denial work.
How urology billing is priced
Percentage of dollars that land — not a flat fee per cystoscopy that ignores bundling leaks.
Written rate
The fee is a percentage of net collections. No setup fee on the monthly cycle. Month to month. The written rate follows office versus surgical mix and how much of the cycle you hand over.
See pricing or request a written rate after the free audit.
What we need to start
EHR access, monthly office and surgical census, top procedures, open A/R if you are leaving another biller, and a sample of recent cystoscopy or biopsy denials. Switching usually overlaps so claim submission does not stop. See EHR integration if access setup needs a separate quote.
Audit the urology claims that unbundle the diagnostic cysto
Send monthly office and surgical volume, top procedures, EHR name, and a sample of bundling or global-period denials. We reply with what the open A/R shows, whether the monthly fee fits, and a written rate — without inventing a clean-claim percentage.
Urology billing questions
Straight answers tied to the sections above.
What does urology billing include?
Office E/M and procedures, surgical professional claims when the case leaves the office, cystoscopy bundling scrub, prostate biopsy under the current CPT family, stone procedures, intravesical drugs, scrubbing, submission, posting, denials, and A/R.
How much do urology billing services cost?
The fee is a percentage of net collections. No setup fee on the monthly cycle. The written rate follows office versus surgical mix and payer mix.
Can 52000 be billed with a therapeutic cystoscopy?
Usually no. Diagnostic cystourethroscopy is an NCCI component of most therapeutic cystoscopy codes the same session. Bill the most comprehensive therapeutic code the note supports.
How is prostate biopsy coded in 2026?
Use 55707–55715 based on approach, imaging modality, and systematic versus targeted sampling. Do not use deleted 55700. Document the imaging method in the operative note.
Does TURP include cystoscopy?
Yes in typical NCCI logic — diagnostic cystoscopy, meatotomy, and urethral dilation the same session are bundled into 52601. Do not unbundle those components.
Do we have to change our EHR?
No. Billers work in the system you already use — see EHR integration. Compatibility is confirmed before you commit.
Where does Vector provide urology billing?
The office is 15717 85th St, Jamaica, NY 11414-2620. Work is remote for urology practices in all 50 states. See the specialties hub.
Is Vector Medical Billing HIPAA certified?
HHS does not certify billing companies. Vector signs a BAA, limits PHI to staff on the account, and follows HIPAA workflows. A HIPAA security risk analysis is a separate engagement when scoped.
