Family Medicine Billing Services
Family medicine billing runs on office E/M, preventive and Medicare wellness visits, and a separately documented problem visit on the same day when the note supports both — plus CCM, TCM, and vaccines that leak when nobody watches the clocks. Vector Medical Billing, in Jamaica, New York, levels visits from MDM or time, keeps wellness and problem work on the right code families, and runs the claim cycle inside the EHR you already use.
What family medicine billing services include
Full-cycle RCM for family medicine clinics, multi-provider primary care offices, and independent primary care groups — visit by visit, month by month for care-management codes.
The operating cycle matches medical billing services: eligibility, charge capture from the encounter, ICD-10 / CPT / HCPCS coding, scrub, clearinghouse submission, payment posting, denial management, and A/R follow-up. The scrub watches E/M level vs MDM or time, preventive vs Medicare AWV selection, same-day modifier 25, CCM/TCM clocks, and vaccine product-plus-admin pairs.
In scope: new and established office/outpatient E/M, commercial preventive medicine by age, Medicare IPPE and AWV, same-day problem-oriented visits when documented, chronic and transitional care management, immunization product and administration, common in-office procedures with correct modifiers, and telehealth when payer POS/modifier rules are current. Pediatrics-heavy vaccine/VFC workflows mirror the same admin logic; dedicated pediatrics accounts can sit on pediatrics billing when scoped separately.
The fee is a percentage of net collections. The written rate is on pricing. Credentialing, a retrospective coding audit, MIPS, and out-of-network / IDR stay separate engagements.
What sits on this family medicine fee
- E/M levels from MDM or time — not a recycled 99213 habit
- Wellness and problem work split when the note supports both
- HIPAA workflows, a signed BAA, and PHI limited to staff on the account
Where family medicine claims fail
Common family medicine billing issues include the wrong wellness code for the payer, a missing modifier 25 on a same-day service, and chronic-care or transitional-care time that was not documented.
Wrong wellness code family
Medicare AWV uses G0438/G0439 (and IPPE G0402). Commercial preventive visits use age-based 99381–99397. Billing G-codes to commercial plans — or commercial preventive codes to Original Medicare — denies or shifts the charge to the patient. Frequency limits (once per 12 months on AWV) add another layer of rejects.
Vector picks the wellness family the payer actually covers before the claim leaves.
Missing modifier 25
Same-day vaccine, injection, minor procedure, or separately identifiable problem visit needs modifier 25 on the E/M when both are payable. Without it, the visit often bundles to zero. The reverse fail is attaching 25 when the note does not show significant, separately identifiable work.
The scrub reads whether the problem section stands apart from the wellness or procedure work.
CCM / TCM clocks missed
CCM dies without consent, a care plan, and monthly time logs. TCM dies when interactive contact is late or the face-to-face visit misses the 7- or 14-day window. Practices often do the clinical work and never bill — or bill after the window has closed.
Vector watches monthly CCM thresholds and post-discharge TCM clocks against the documentation on file.
Office E/M — level from the note, not the habit
New and established outpatient visits select level by medical decision making or total time on the date of the encounter.
Under current office/outpatient guidelines, 99202–99215 no longer rely on history and exam bullet counts for leveling. Coders choose MDM (problems, data, risk) or documented total time — whichever the note supports. Chronic problem lists that never speak to today’s data or risk keep practices stuck at a default mid-level; the reverse — a 99214 share far above the specialty pattern without documentation — invites payer review.
Vector levels from what is written that day. Diagnosis pointers must support medical necessity for the problems managed. When a minor procedure or vaccine shares the date, the E/M still needs its own separately identifiable story if modifier 25 is used.
What the scrub checks on office visits
- MDM elements or time minutes support the CPT level
- New vs established patient status is correct
- Same-day procedures carry 25 only when the visit stands alone
Preventive visit, AWV, and same-day problem care
Bill the office visit, the preventive visit, and a same-day problem visit only when the note supports each one.
Commercial preventive
Age-based preventive medicine codes (99381–99387 new; 99391–99397 established) for plans that cover an annual physical. Code must match the patient’s age band. Frequency and diagnosis rules are payer-specific.
Medicare AWV / IPPE
G0402 Welcome to Medicare, G0438 initial AWV, G0439 subsequent AWV. Required elements (HRA, personalized prevention plan, depression and functional screening as applicable) must appear. Do not bill G0438/G0439 within 12 months of each other incorrectly, or within 12 months of G0402 for the same patient when CMS frequency rules block it. Do not bill AWV and a CPT preventive physical on the same date.
Same-day problem visit
When acute or chronic problem management is medically necessary during a wellness visit, bill the office E/M with modifier 25. Count only problem-oriented MDM or problem-specific time toward the E/M — not the wellness components. Document the problem work in a section that stands apart from the wellness checklist.
Chronic care and transitional care management
Monthly and post-discharge clocks — billed only when consent, plan, and time (or contact) are on file.
CCM (99490 family)
Two or more qualifying chronic conditions, patient consent, a personalized care plan, an initiating visit when required, and monthly clinical-staff or practitioner time thresholds. Only one practitioner bills CCM for a beneficiary in a given month. Complex CCM has its own time and MDM rules.
TCM (99495 / 99496)
Interactive contact within 2 business days after discharge (or documented unsuccessful attempts). Face-to-face visit within 14 days for moderate MDM (99495) or 7 days for high MDM (99496). Miss the visit window and TCM cannot be billed — the contact clock and the visit clock are not the same rule.
Initiating visit & conflicts
CCM needs a qualifying initiating visit for new or long-unseen patients. TCM and certain care-management codes conflict in the same period under CMS rules — the scrub checks which program the month actually supports before stacking.
Vaccines and same-day injections
Product plus administration — and an E/M with 25 only when the visit is separately identifiable.
Product and admin pairs
Each immunization needs the vaccine product code and the correct administration code (for example 90471/90472, or counseling-based pediatric admin families when used). Medicare often uses G-codes for certain adult vaccine administration. Product without admin — or admin without product — is a routine denial. Combo vaccines and VFC (administration-only with program modifiers) need their own route.
Vector matches NDC/product, route, and admin code to what was given that day.
Eligibility and Medicaid churn
Primary care carries a heavy Medicaid/CHIP mix. Coverage that changes mid-month produces “coverage terminated” denials on visits that felt covered at check-in. Re-verification before high-cost vaccines and recurring monthly CCM claims catches churn that a once-a-year eligibility check misses.
See eligibility verification when front-end checks need a separate scope.
Family medicine code families this service works
Illustrative anchors — the note and payer policy decide the final claim.
| Work type | Example | What to check | Common failure if skipped |
|---|---|---|---|
| Office / outpatient E/M | 99202–99215 | MDM or time; new vs established | Default mid-level every visit |
| Commercial preventive | 99381–99397 | Age band; payer frequency | Wrong age code; billed to Medicare FFS |
| Medicare wellness | G0402; G0438; G0439 | Required elements; 12-month rules | G-code to commercial; early repeat AWV |
| Same-day problem | E/M + 25 | Separately identifiable documentation | Bundled visit; unsupported 25 |
| CCM | 99490 / 99439 / 99491… | Consent; plan; monthly minutes | Work done, never billed |
| TCM | 99495 / 99496 | 2-business-day contact; 7/14-day visit | Window missed; clocks confused |
| Vaccines | Product + admin (± Medicare G) | Pair; VFC rules; same-day 25 | Product without admin |
This page does not invent a family medicine denial percentage. The free audit reads your wellness mix-ups, 25 gaps, and missed CCM/TCM on the sample you send.
What a free audit line looks like
Illustrative composite only — not a client result and not a promised recovery.
| Field | Example entry |
|---|---|
| Documented service | Medicare established patient: subsequent AWV elements completed; separately documented moderate MDM for uncontrolled hypertension and medication change; flu vaccine product and admin given same day |
| Billed codes | 99396 to Medicare; 99214 without 25; vaccine product only (no admin) |
| Issue found | Commercial preventive code not payable under Original Medicare → use G0439; problem E/M needs 25; admin code missing for vaccine |
| Recommended correction | G0439 + 99214-25 (problem section supports moderate MDM) + vaccine product + correct admin (or Medicare G-admin when applicable) |
| Related check | Confirm last AWV/IPPE date for 12-month frequency |
| Why it matters | Wrong wellness family + missing 25 + incomplete vaccine pair on one high-volume day type — caught in scrub or flagged in the free audit |

How Vector runs family medicine billing
Specialty assignment and visit-type reconciliation first — then the same revenue cycle.
Assign primary care
Coder and biller who already work E/M, AWV vs preventive, 25, CCM/TCM, and vaccines.
Reconcile and code
Charge entry and coding from the note — queries when level, wellness, or time cannot be supported.
Post and report
835 posting, denial work, A/R aging, monthly RCM report by provider and visit type.
Not inside the family medicine percentage
Related work stays separate so the quote matches the job.
Credentialing
Provider credentialing for physicians and NPPs across commercial and Medicare Advantage panels is a separate project.
Coding audit
A retrospective billing and coding audit of E/M levels, wellness mix, and 25 use is scoped on its own when you want a period review.
Underpay / OON
Underpayment recovery and OON / IDR are separate when the dispute path is not ordinary denial work.
How family medicine billing is priced
Percentage of dollars that land — not a flat fee per visit that ignores wellness and CCM leaks.
Written rate
The fee is a percentage of net collections. No setup fee on the monthly cycle. Month to month. The written rate follows visit volume, payer mix, and how much of the cycle you hand over.
See pricing or request a written rate after the free audit.
What we need to start
EHR access, monthly visit volume and payer mix, open A/R if you are leaving another biller, and a sample of recent wellness, 25, or CCM denials. Switching usually overlaps so claim submission does not stop. See EHR integration if access setup needs a separate quote.
Audit the family medicine claims that are already leaking
Send monthly visit volume, top payers, EHR name, and a sample of wellness or modifier-25 denials. We reply with what the open A/R shows, whether the monthly fee fits, and a written rate — without inventing a clean-claim percentage.
Family medicine billing questions
Straight answers tied to the sections above.
What does family medicine billing include?
Office E/M leveling, commercial preventive visits, Medicare AWV/IPPE, same-day problem visits with modifier 25 when supported, CCM, TCM, vaccine product and administration, claim scrubbing, submission, payment posting, denial management, and A/R follow-up.
How much do family medicine billing services cost?
The fee is a percentage of net collections. No setup fee on the monthly cycle. The written rate follows visit volume and payer mix.
Can you bill a wellness visit and a problem visit the same day?
Yes when both are medically necessary and separately documented. Bill the problem-oriented office E/M with modifier 25 alongside the preventive or Medicare AWV code. Do not bill a Medicare AWV and a commercial preventive physical on the same date.
What is the difference between an AWV and a preventive physical?
Medicare pays AWV with G0438/G0439 (and IPPE G0402). Commercial plans typically pay preventive medicine CPT codes by age. Mixing those families across the wrong payer is a common denial.
Do you bill CCM and TCM?
Yes when documentation supports them: CCM needs conditions, consent, care plan, and monthly time; TCM needs timely interactive contact and a face-to-face visit within the 7- or 14-day window for the code billed.
Do we have to change our EHR?
No. Billers work in the system you already use — see EHR integration. Compatibility is confirmed before you commit.
Where does Vector provide family medicine billing?
The office is 15717 85th St, Jamaica, NY 11414-2620. Work is remote for family medicine practices in all 50 states. See the specialties hub.
Is Vector Medical Billing HIPAA certified?
HHS does not certify billing companies. Vector signs a BAA, limits PHI to staff on the account, and follows HIPAA workflows. A HIPAA security risk analysis is a separate engagement when scoped.
