Pediatrics Billing Services
Pediatrics billing runs on well-child visits by age, vaccine administration — including counseling components and Vaccines for Children stock — and a sick visit on the same day when both are documented. Vector Medical Billing, in Jamaica, New York, matches preventive codes to date of birth, pairs product with admin, and runs the claim cycle inside the EHR you already use.
What pediatrics billing services include
Full-cycle RCM for pediatric groups, multi-provider kids’ clinics, and practices with a heavy Medicaid/CHIP panel — well visits, shots, and sick visits on the same calendar.
The operating cycle matches medical billing services: eligibility (including plan changes and newborn member IDs), charge capture from the encounter, ICD-10 / CPT coding, scrub, clearinghouse submission, payment posting, denial management, and A/R follow-up. The scrub watches age band vs date of birth, vaccine product-plus-admin pairs, VFC stock rules, same-day sick-visit modifier 25, and screening tool documentation.
In scope: infant through adolescent preventive medicine, immunizations, developmental and behavioral screenings, vision and hearing screens, fluoride varnish when documented, after-hours codes when the payer pays them, and newborn hospital care when it sits on the same account. Adult family-medicine wellness and AWV depth sits on family medicine billing and primary care billing when that is the better fit.
The fee is a percentage of net collections. The written rate is on pricing. Credentialing, a retrospective coding audit, MIPS, and out-of-network / IDR stay separate engagements.
What sits on this pediatrics fee
- Well-child code matched to age — not a recycled mid-child visit
- Vaccine counseling vs VFC admin path checked before submission
- HIPAA workflows, a signed BAA, and PHI limited to staff on the account
Where pediatrics claims fail
Patterns AAP preventive coding, Medicaid VFC rules, and specialty pediatric RCM vendors keep naming.
Age band vs date of birth
Preventive medicine codes split by age (infant, early childhood, late childhood, adolescent) and by new vs established. A code that does not match the child’s age on the date of service denies or pays the wrong preventive benefit. Well-child codes are not time-based — extra minutes do not bump the preventive code.
Vector selects the preventive family from date of birth, then checks new vs established status.
Vaccine product vs VFC stock
Private-stock doses need product plus administration. VFC doses supplied for eligible children are generally administration-only — billing the product to Medicaid is an audit recoupment, not a clean claim. Counseling-based 90460/90461 require documented face-to-face counseling by the physician or qualified professional; otherwise use the non-counseling admin family. NDC must match the vial used when the plan requires it.
The scrub asks which fridge the dose came from before the product line is added.
Well visit plus sick visit
A significant illness addressed during a well visit is a separate office E/M with modifier 25 on the problem code when MDM or time supports it. An insignificant finding discovered during the exam is not. Missing 25 bundles the sick work; automatic 25 on every well day invites review.
Vector bills the second line only when the problem section can stand alone.
Well-child visits — age, not minutes
Hub language made operational: the well visit, the shots, and the sick visit when both are documented.
AAP preventive coding treats measurements, age-appropriate history, and examination as the preventive medicine service (new 99381–99385 and established 99391–99395 for pediatric age bands). Time spent does not select that code. Immunizations, labs, and separately coded screenings are reported in addition to the preventive visit.
If an illness is significant enough to require problem-oriented E/M components, report 99202–99215 with modifier 25 alongside the well-child code. Example pattern from AAP guidance: established early-childhood preventive plus a moderate problem visit as 99392 and 99213-25. Trivial problems stay inside the well visit.
What the scrub checks on a well day
- Preventive code matches age band and new vs established status
- Diagnosis fits the exam (without vs with abnormal findings)
- Sick-visit 25 only when problem MDM is separately documented
Vaccine administration and VFC
Counseling components, private stock, and state-supplied stock are three different claim paths.
90460 / 90461
Through age 18, when counseling is provided by a physician or other qualified professional, 90460 is the first or only component of each vaccine and 90461 is each additional component. A component is antigens against one organism — combination vaccines can carry more than one component. Conjugates and adjuvants are not extra components under that definition.
Without counseling
When counseling rules are not met, or the patient is outside the counseling-age path, use the non-counseling administration family (90471–90474 routes). Product codes still apply for privately purchased vaccine. Do not mix counseling and non-counseling admin codes for the same component without a reason the note supports.
VFC admin-only
Vaccines for Children covers eligible children (Medicaid-enrolled, uninsured, underinsured, and other groups the program lists) with federally purchased product. Bill administration — often with a state-supplied modifier from that Medicaid manual — and do not charge Medicaid for the product. Keep VFC and private inventory separate so a private product line never lands on a VFC child.
Screenings, EPSDT, and the state schedule
A named tool and a score — and the periodicity schedule the payer actually adopted.
96110 / 96127
Developmental screening and brief emotional/behavioral assessment are separately reportable when a standardized instrument is named, scored, and interpreted. Units follow instruments administered. A blank “screening done” line does not survive review.
Vision, hearing, fluoride
Visual acuity, pure-tone hearing, instrument-based ocular screening, and fluoride varnish have their own codes and age/frequency rules. Some plans bundle them into the preventive allowance; others pay them separately. Vector follows the contract, not a national habit.
EPSDT / Medicaid schedule
For Medicaid, the state’s adopted periodicity schedule governs when a screening is expected — not only a national Bright Futures calendar. Well-child diagnosis coding distinguishes exams without abnormal findings from those with abnormal findings. Eligibility and managed-care plan changes mid-month are a leading pediatric denial source.
Modifier 25 on a pediatric day
Sick visit, vaccines, and NCCI edits do not share one automatic modifier habit.
Sick visit beside the well visit
AAP guidance appends modifier 25 to the problem-oriented office code, not to the preventive code, when both are reported. The problem note must show history of the complaint and medical decision making (or time) beyond the preventive exam. Parents should know the sick portion may carry cost-sharing even when the well visit does not.
See also primary care billing for the same two-claim principle on adult wellness days.
Vaccines and NCCI
Some NCCI edits treat preventive E/M as a component of immunization administration unless a modifier shows they are separate. Many commercial and Medicaid plans expect modifier 25 on the preventive visit when vaccines are also billed. Vector applies the modifier the payer’s edit actually requires — documented counseling or a distinct visit — instead of one national sticker on every line.
NDC, route, and component count are checked on the same pass.
Medicaid, CHIP, and newborn IDs
Pediatric denials often start at eligibility — wrong plan, no baby ID, or VFC status unknown.
Children move between Medicaid managed-care plans and CHIP more often than adult commercial panels. A claim sent to last month’s plan denies even when the visit was clinically correct. Newborn hospital and early office claims filed before the infant’s own member ID exists bounce until demographics are updated.
Vector re-checks coverage before high-vaccine days and flags newborn accounts waiting on an ID. See eligibility verification when front-end checks need a separate scope.
Front-end controls
- Active Medicaid/CHIP plan confirmed for the date of service
- VFC eligibility vs private stock decided before charge entry
- Newborn claims held until a member ID exists
Pediatrics code families this service works
Illustrative anchors — the note, age, stock source, and payer policy decide the final claim.
| Work type | Example | What to check | Common failure if skipped |
|---|---|---|---|
| Well-child | 99381–99385 / 99391–99395 | Age band; new vs established | Wrong age code; time used to “upgrade” |
| Same-day sick | 99202–99215 + 25 | Significant problem MDM | Trivial finding billed; missing 25 |
| Counseling admin | 90460 / 90461 | Face-to-face counseling; components | Combo vaccine under-counted |
| Non-counseling admin | 90471–90474 | When counseling path does not apply | Wrong admin family |
| VFC | Admin ± state modifier | No product charge; correct stock | Private product billed on VFC dose |
| Screenings | 96110 / 96127; vision/hearing | Tool, score, interpretation; state schedule | “Screening done” with no instrument |
| Newborn hospital | 99460–99463 path | Setting; subsequent days; member ID | Claim before infant ID issued |

What a free audit line looks like
Illustrative composite only — not a client result and not a promised recovery.
| Field | Example entry |
|---|---|
| Documented service | Established 15-month well visit; standardized developmental tool named with score; VFC combination vaccine with physician counseling; separately documented otitis with antibiotic prescribed |
| Billed codes | Adolescent preventive code; private vaccine product charged to Medicaid; no 90460; no sick E/M; “screening” with no instrument name |
| Issue found | Age band wrong; VFC product should not be billed; counseling admin components missing; significant sick visit not billed with 25; 96110 unsupported until tool/score are on the claim story |
| Recommended correction | Age-correct established preventive; 96110 with tool documented; VFC admin (90460/90461 components as documented) without product charge; 99213-25 for the otitis MDM |

How Vector runs pediatrics billing
Age, stock, and same-day sick documentation first — then the same revenue cycle.
Assign pediatrics
Coder and biller who already work well-child bands, 90460, VFC, and EPSDT schedules.
Reconcile and code
Charge entry and coding from the note and vaccine log — queries when counseling or screening cannot be supported.
Not inside the pediatrics percentage
Related work stays separate so the quote matches the job.
Credentialing
Provider credentialing for pediatricians and NPPs across Medicaid and commercial panels is a separate project.
Coding audit
A retrospective billing and coding audit of well visits, vaccines, and 25 use is scoped on its own when you want a period review.
Underpay / OON
Underpayment recovery and OON / IDR are separate when the dispute path is not ordinary denial work.
How pediatrics billing is priced
Percentage of dollars that land — not a flat fee per well visit that ignores VFC and screening leaks.
Written rate
The fee is a percentage of net collections. No setup fee on the monthly cycle. Month to month. The written rate follows visit volume, Medicaid mix, and how much of the cycle you hand over.
See pricing or request a written rate after the free audit.
What we need to start
EHR access, monthly well and sick volume, Medicaid/VFC share, open A/R if you are leaving another biller, and a sample of recent vaccine or age-band denials. Switching usually overlaps so claim submission does not stop. See EHR integration if access setup needs a separate quote.
Audit the pediatric claims that miss shots, age, or the sick visit
Send monthly visit volume, Medicaid share, EHR name, and a sample of vaccine or well-visit denials. We reply with what the open A/R shows, whether the monthly fee fits, and a written rate — without inventing a clean-claim percentage.
Pediatrics billing questions
Straight answers tied to the sections above.
What does pediatrics billing include?
Age-based well-child coding, vaccine product and administration including counseling codes and VFC rules, same-day sick visits with modifier 25 when documented, developmental and sensory screenings when the note supports them, Medicaid/CHIP claim rules, scrubbing, submission, posting, denials, and A/R follow-up.
How much do pediatrics billing services cost?
The fee is a percentage of net collections. No setup fee on the monthly cycle. The written rate follows visit volume and payer mix.
Can you bill a well visit and a sick visit the same day?
Yes when the illness requires problem-oriented MDM or time beyond the preventive visit. Bill the age-appropriate well-child code and the office E/M with modifier 25 on the problem visit. Trivial findings are not a second claim.
How are pediatric vaccines billed?
Product plus administration for private stock. Through age 18 with documented counseling, 90460/90461 by component. VFC doses are generally administration-only — do not bill the product to Medicaid when the stock was state-supplied.
What supports developmental screening?
Name the standardized instrument, record the score, and document interpretation. State Medicaid periodicity schedules decide whether that age is separately payable.
Do we have to change our EHR?
No. Billers work in the system you already use — see EHR integration. Compatibility is confirmed before you commit.
Where does Vector provide pediatrics billing?
The office is 15717 85th St, Jamaica, NY 11414-2620. Work is remote for pediatric practices in all 50 states. See the specialties hub.
Is Vector Medical Billing HIPAA certified?
HHS does not certify billing companies. Vector signs a BAA, limits PHI to staff on the account, and follows HIPAA workflows. A HIPAA security risk analysis is a separate engagement when scoped.
