Occupational Therapy Billing Services
Occupational therapy billing uses the same timed-unit rules as physical therapy — with the plan of care the payer asks for. Vector Medical Billing, in Jamaica, New York, converts ADL minutes to units, applies GO and CO, and holds visits that fall outside a current OT plan inside the EHR you already use.
What occupational therapy billing services include
Full-cycle RCM for OT clinics and hospital outpatient OT — timed units plus the plan of care, not a physical therapy claim with a different label.
The operating cycle matches medical billing services: eligibility, charge capture from the daily note, ICD-10 / CPT coding, scrub, clearinghouse submission, payment posting, denial management, and A/R follow-up. The scrub watches 8-minute totals, untimed evaluations, modifier GO, assistant modifier CO, plan-of-care dates, and the OT therapy threshold.
In scope: OT evaluation and re-evaluation, therapeutic activities, self-care and home-management training, therapeutic exercise and other timed procedures when an occupational therapist or OTA provides them under an OT plan, and group OT when the code and minutes support it. The shared minute math is the same discipline as physical therapy billing, but the modifier is GO, not GP, and the assistant modifier is CO, not CQ. Speech-language pathology and chiropractic manipulation stay off this page.
Occupational therapy is priced as a percentage of net collections with other timed therapy work. The written rate follows visit volume and payer mix — see pricing. Credentialing, a retrospective coding audit, MIPS, and out-of-network / IDR stay separate engagements.
What sits on this OT fee
- Timed units from documented one-on-one minutes
- GO, CO, and a current plan of care checked before submission
- HIPAA workflows, a signed BAA, and PHI limited to staff on the account
Where occupational therapy claims fail
Patterns Medicare therapy billing and OT modifier guidance keep naming.
GP on an OT line
Outpatient occupational therapy submitted with the physical therapy modifier. Or CO paired with GP. Medicare returns those pairings as unprocessable.
Hub rule: same timed units — the discipline modifier is still GO.
Units that the minutes do not support
An evaluation billed in 15-minute units. A 7-minute timed activity billed as a unit. Two timed codes each given a full unit when the day’s total only supports one. OTA minutes ignored, so CO never appears.
Total the timed minutes, then assign units. Untimed stays one.
Visits after the plan expires
Clean units on a date past the certified plan of care, or with no signed OT plan on file. KX stamped on every visit, including those still under the OT threshold.
The payer asked for a current plan. Units do not replace it.
The same timed-unit rules
Hub language made operational: Medicare’s 8-minute bands, applied to OT minutes — not copied from the PT note.
Timed OT codes are defined in 15-minute blocks of direct one-on-one care. For a single timed code, one unit is 8 through 22 minutes, two units are 23 through 37, three are 38 through 52, and the pattern continues. Do not bill a timed unit for fewer than 8 minutes. When several timed codes are done the same day, Medicare totals the timed minutes first, then assigns that many units to the codes that consumed the time. Seven minutes of one code plus seven of another can support one unit on the longer service — not a unit on each.
Some commercial contracts follow a per-code midpoint instead of Medicare’s daily total. The physical therapy page walks through that split. OT claims use the rule that payer actually applies. An OT evaluation (97165, 97166, or 97167) and a re-evaluation (97168) are untimed: one unit for the service, however long the eval took, chosen by the complexity the note supports.
Unit checklist
- Direct one-on-one minutes, not charting time
- Daily total decides how many timed units exist
- Eval and re-eval stay one unit
The plan of care the payer asks for
A correct unit count on an expired OT plan still denies.
What the plan has to contain
Medicare outpatient OT expects a plan of care with the diagnosis, long-term treatment goals that are functional, the type and amount of therapy, and the frequency and duration. A physician or other qualified practitioner certifies it. Treatment after the certification window needs a recertification — commonly no later than 90 days, and sooner when the plan itself is shorter. Commercial and Medicaid plans often add visit caps or prior authorization on top of that plan.
KX on the OT threshold
Medicare tracks an annual OT threshold that is separate from the physical therapy and speech-language threshold. Modifier KX on an OT line says continued skilled therapy above that threshold is still medically necessary and the record supports it. KX is not a stamp for every visit under the threshold, and it does not guarantee payment. We do not print a dollar cap here — the amount changes by year, and the scrub uses the figure in effect for that date of service.
GO, CO, and the codes OT actually uses
The assistant modifier follows the occupational therapy assistant, not the physical therapist assistant.
GO with CO
GO identifies outpatient occupational therapy and belongs on the OT treatment and eval lines that payer requires. CO identifies services furnished in whole or in part by an OTA. Report CO with GO. When the OTA’s independent minutes exceed 10 percent of the service or unit, CO applies — with the two Medicare exceptions for a final unit the OT personally meets at 8 minutes, and for the two-unit split when each clinician is in the 9-to-14-minute band. Minutes the OT and OTA furnish together count as the OT.
Eval and ADL codes
97165, 97166, and 97167 are low, moderate, and high OT evaluations. 97168 is the re-evaluation. 97530 is therapeutic activities. 97535 is self-care and home-management training — dressing, meal prep, and the other daily tasks that define a lot of OT. 97110 and similar exercise codes are timed the same way when the OT plan includes them. Do not bill an OT eval and a PT eval as if they were one discipline.
Who may render
The rendering clinician has to be the occupational therapist or the OTA under that payer’s supervision rules, enrolled the way the plan requires. A PTA’s CQ modifier does not convert a line into OT. Enrollment gaps are a credentialing project, not a unit rewrite.
Occupational therapy code families this service works
Illustrative anchors — the note and the plan of care decide the line.
| Work type | Example | What to check | Common failure if skipped |
|---|---|---|---|
| OT evaluation | 97165 / 97166 / 97167 | Complexity in the note; 1 unit | Billed in 15-minute units |
| Re-evaluation | 97168 | Why a re-eval, not a treatment | Habit add-on |
| Therapeutic activities | 97530 × units | Direct minutes | Units past the 8-minute total |
| Self-care / ADL | 97535 × units | Daily-task training documented | Exercise code for dressing practice |
| Discipline | GO | Outpatient OT | GP on an OT claim |
| OTA | CO with GO | De minimis / 8-minute exceptions | CQ used by mistake |
| Threshold | KX | OT threshold crossed; necessity documented | KX on every visit |
| Plan of care | Certified OT plan | Dates cover the visit | Units billed after expiry |
This page does not invent an occupational therapy denial percentage. The free audit reads your unit totals, GO/CO pairs, and plan dates on the sample you send.
What a free audit line looks like
Illustrative composite only — not a client result and not a promised recovery.
| Field | Example entry |
|---|---|
| Documented service | OT provides 20 minutes of 97535 (dressing and meal setup). OTA independently provides 18 minutes of 97530. Same day. Plan of care is certified and current. |
| Billed codes | 97535 × 2 and 97530 × 2, modifier GP, no CO |
| Issue found | 38 timed minutes support 2 units total, not 4; discipline modifier is GP instead of GO; OTA time has no CO |
| Recommended correction | Bill one unit of 97535-GO (OT’s 20 minutes) and one unit of 97530-GO-CO (OTA’s independent 18 minutes) |
| Related check | KX only if this patient’s OT charges have crossed that year’s OT threshold |
| Why it matters | Extra units and the wrong discipline modifier deny or recoup — caught in scrub or flagged in the free audit |

How Vector runs occupational therapy billing
Minutes and the plan date first — then the same revenue cycle.
Assign OT RCM
Coder and biller who already separate GO/CO from the physical therapy pair.
Reconcile and code
Charge entry and coding from the daily note — queries when minutes or the plan date are missing.
Not inside the occupational therapy percentage
Related work stays separate so the quote matches the job.
Facility therapy charges
Hospital or SNF facility charges stay on the facility claim. We bill the professional OT services your practice owns.
Credentialing / audit
Credentialing and a retrospective coding audit of units and CO usage are separate engagements when scoped.
Underpay / OON
Underpayment recovery and OON / IDR stay separate when the dispute path is not ordinary denial work.
How occupational therapy billing is priced
Percentage of dollars that land — not a flat fee per visit that ignores unit recoupment.
Written rate
Occupational therapy is priced as a percentage of net collections because timed units, assistant modifiers, and plan-of-care follow-up take more scrub time. No setup fee on the monthly cycle. Month to month. The written rate follows visit volume and how much of the cycle you hand over.
See pricing or request a written rate after the free audit.
What we need to start
EHR access, how often an OTA treats, open A/R if you are leaving another biller, and a sample of unit, GO, or plan-of-care denials. Switching usually overlaps so claim submission does not stop. See EHR integration if access setup needs a separate quote.
Audit the OT claims that still carry a PT modifier
Send monthly visit volume, whether OTAs treat, EHR name, and a sample of unit or plan-of-care denials. We reply with what the open A/R shows, whether the monthly fee fits, and a written rate — without inventing a clean-claim percentage.
Occupational therapy billing questions
Straight answers tied to the sections above.
What does occupational therapy billing include?
Timed and untimed OT coding, GO and CO when required, plan-of-care and KX threshold scrub, submission, posting, denials, and A/R.
How much do occupational therapy billing services cost?
The fee is a percentage of net collections. No setup fee on the monthly cycle. The written rate follows visit volume and payer mix.
Does OT use the same 8-minute rule as physical therapy?
Yes for Medicare and for plans that follow it. One unit is 8 through 22 minutes of a timed code; two units are 23 through 37. Evaluations are untimed. Some commercials use a per-code midpoint instead.
What is modifier GO?
GO marks outpatient occupational therapy. CO marks an OTA’s qualifying minutes and is reported with GO. GP and CQ belong on physical therapy lines.
Do we have to change our EHR?
No. Billers work in the system you already use — see EHR integration. Compatibility is confirmed before you commit.
Where does Vector provide occupational therapy billing?
The office is 15717 85th St, Jamaica, NY 11414-2620. Work is remote for OT practices in all 50 states. See the specialties hub.
Is Vector Medical Billing HIPAA certified?
HHS does not certify billing companies. Vector signs a BAA, limits PHI to staff on the account, and follows HIPAA workflows. A HIPAA security risk analysis is a separate engagement when scoped.
