ABA Therapy Billing Services
ABA therapy billing is timed behavior-analysis codes and the supervising analyst — units are time, not a visit level. Vector Medical Billing, in Jamaica, New York, converts documented active minutes into 15-minute units and keeps technician and QHP lines from colliding inside the EHR you already use.
What ABA therapy billing services include
Full-cycle RCM for applied behavior analysis practices — not an office-visit queue with a different specialty label.
The operating cycle matches medical billing services: eligibility, prior authorization for assessment and treatment hours, charge capture from session notes with start and stop times, ICD-10 / CPT coding, scrub, clearinghouse submission, payment posting, denial management, and A/R follow-up. The scrub watches 15-minute unit math from active minutes, supervising QHP versus technician rendering, concurrent-code combinations, and remaining authorized units.
In scope: behavior identification assessment and supporting assessment, adaptive behavior treatment by protocol and with protocol modification, group treatment, family adaptive behavior treatment guidance, and payer-required modifiers that identify the rendering level. Speech, OT, and PT overlapping the same clock are out of scope as concurrent ABA time. Dedicated psychotherapy, psychiatry, SUD, and peer-support pages cover those programs separately.
ABA therapy is priced as a percentage of net collections with other timed behavioral-health work. The written rate follows authorized weekly hours and payer mix — see pricing. Credentialing of BCBAs and technicians, a retrospective coding audit, MIPS, and out-of-network / IDR stay separate engagements.
What sits on this ABA fee
- Timed units built from documented active minutes
- Supervising analyst and technician lines scrubbed for concurrency
- HIPAA workflows, a signed BAA, and PHI limited to staff on the account
Where ABA therapy claims fail
Patterns ABA coding FAQs and state documentation tips keep naming.
Block billing the whole day
One start–stop from drop-off to pick-up that includes lunch, nap, and idle time. Units rounded up on each short segment before summing. Multiple claim lines for the same code on the same day that look like duplicates.
Hub rule: units are time — only active service minutes.
Wrong code for who was in the room
Technician protocol time billed as 97155. QHP protocol modification billed as 97153. One clinician stacking 97153 and 97155 for the same minutes. Group codes used for 1:1 work.
The supervising analyst owns direction; the descriptor owns the CPT.
Auth and concurrency gaps
Treatment units that outrun the prior authorization. Assessment hours past the plan’s MUE without clinical support. ABA overlapping the same clock as speech, OT, or PT when the payer forbids it.
Authorized units and concurrent rules decide what can post.
Timed codes and the supervising analyst
Hub language made operational: 15-minute units from active minutes, under a qualified health care professional’s direction.
ABA adaptive-behavior CPT codes are timed. They are not E/M visit levels. Sum all active minutes for a given code across the day, subtract naps, meals, free play, and administrative downtime, then convert once to 15-minute units. Recording trial data during protocol delivery can be billable; writing the progress note after the session ends usually is not. 97151 is the QHP behavior identification assessment and may include limited non-face-to-face analysis and plan preparation within that code’s rules — it is still timed, not a flat intake fee.
Technicians deliver protocol treatment under the direction of a physician or other QHP (commonly a BCBA). Claims typically bill under the supervising analyst’s enrollment, with the rendering role identified by the code and any payer-required modifiers. Direction without the patient present is not a separate technician treatment line.
Unit checklist
- Exact start and stop for each active segment
- Non-therapy time subtracted before unit conversion
- One daily line per code when the payer rejects split duplicates
Assessment versus treatment codes
Who administered the service decides the CPT — not the appointment length alone.
97151 and 97152
97151 is assessment by the QHP, face-to-face with the patient and/or caregivers plus allowed non-face-to-face analysis and report/plan work. 97152 is supporting assessment by a technician under QHP direction, face-to-face with the patient. Do not bill technician supporting assessment as the QHP assessment code. Many payers cap daily or annual assessment units; excess often needs clinical justification beyond a default MUE.
97153 and 97155
97153 is adaptive behavior treatment by protocol, administered by a technician under QHP direction. 97155 is treatment with protocol modification by the QHP, which may include simultaneous direction of a technician while the patient is present. When both occur in overlapping minutes, different providers must deliver each service and each descriptor must be met. Do not bill 97153 separately for the same minutes the QHP is already directing under 97155 as a single-provider stack.
Group, family, concurrency, and authorization
Same-day ABA is allowed. Same-clock collisions are not — unless the descriptors and staffing allow them.
Group and family codes
97154 is group treatment by protocol (technician). 97158 is group treatment with protocol modification (QHP). 97156 is family adaptive behavior treatment guidance by the QHP, with or without the patient present. Do not use a 1:1 code for a group session, or a group code for individual work.
Overlapping clocks
Many payers forbid ABA overlapping the same block as speech, OT, or PT. 97153 with 97156 can be concurrent when two staff deliver distinct services (child protocol and caregiver guidance). Follow that payer’s concurrency table — the free audit maps your combinations, not a single national denial rate.
Prior auth units
Assessment and treatment hours are commonly authorized in advance. Track remaining units by code family so claims do not outrun the auth. Authorization is not a payment guarantee — documentation, timely filing, and enrollment still apply. Some plans also require TG/TF or technician modifiers; bill what that network lists.
ABA code families this service works
Illustrative anchors — documented active minutes decide the units.
| Work type | Example | What to check | Common failure if skipped |
|---|---|---|---|
| QHP assessment | 97151 | QHP time; report/plan rules | Billed as a visit level |
| Tech supporting assessment | 97152 | Under QHP direction | Billed as 97151 |
| Tech protocol treatment | 97153 | Active 1:1 minutes | Nap/lunch in the units |
| Group by protocol | 97154 | ≥2 patients; tech | 1:1 billed as group |
| QHP protocol modification | 97155 | QHP face-to-face | Same person + 97153 overlap |
| Family guidance | 97156 | Caregiver face-to-face | Status update as therapy |
| Group with modification | 97158 | QHP-led group | Stacked with 97154 wrong |
| Authorization | Weekly / code caps | Remaining units | Units past the auth |
This page does not invent an ABA denial percentage. The free audit reads your unit math and concurrent pairs on the sample you send.
What a free audit line looks like
Illustrative composite only — not a client result and not a promised recovery.
| Field | Example entry |
|---|---|
| Documented service | RBT delivers protocol treatment 8:30–11:30 and 1:00–2:30; lunch and nap 11:30–1:00 with no BIP targets; BCBA on site 1:00–2:00 modifying protocol and directing the technician with the patient present |
| Billed codes | 97153 × 32 units (8:30–4:00 block) plus 97155 × 4 units by the same rendering NPI for 1:00–2:00 |
| Issue found | Non-therapy time included; 97153 not reduced for QHP-directed overlap; single-provider concurrency |
| Recommended correction | 97153 for 270 active minutes = 18 units on one line; 97155 × 4 under the BCBA for 1:00–2:00; do not bill 97153 for those same 60 minutes under one clinician |
| Related check | Auth still has weekly 97153/97155 units; no OT overlap on the same clock |
| Why it matters | Block-billed days and illegal concurrency deny or recoup — caught in scrub or flagged in the free audit |

How Vector runs ABA therapy billing
Active minutes and who was in the room first — then the same revenue cycle.
Assign ABA RCM
Coder and biller who already convert session clocks into 9715x units.
Reconcile and code
Charge entry and coding from start–stop notes — queries when concurrency is unclear.
Post and report
835 posting, denial work, A/R aging, monthly RCM report by assessment versus treatment units.
Not inside the ABA therapy percentage
Related work stays separate so the quote matches the job.
School LEA / IEP billing
School-district Medicaid claiming under an IEP path is a different enrollment and claim flow. We bill the clinic or agency ABA claims your practice owns.
Credentialing / audit
Credentialing of BCBAs and technicians, and a retrospective coding audit of unit math, are separate when scoped.
Underpay / OON
Underpayment recovery and OON / IDR stay separate when the dispute path is not ordinary denial work.
How ABA therapy billing is priced
Percentage of dollars that land — not a flat fee per day that ignores unauthorized units.
Written rate
ABA therapy is priced as a percentage of net collections because timed units, supervision, and authorization tracking take more scrub and follow-up time. No setup fee on the monthly cycle. Month to month. The written rate follows authorized hours and how much of the cycle you hand over.
See pricing or request a written rate after the free audit.
What we need to start
EHR or practice-management access, current authorizations by code, open A/R if you are leaving another biller, and a sample of unit or concurrency denials. Switching usually overlaps so claim submission does not stop. See EHR integration if access setup needs a separate quote.
Audit the ABA days that still bill lunch and nap as units
Send monthly authorized hours, typical day schedules, EHR name, and a sample of unit or concurrent denials. We reply with what the open A/R shows, whether the monthly fee fits, and a written rate — without inventing a clean-claim percentage.
ABA therapy billing questions
Straight answers tied to the sections above.
What does ABA therapy billing include?
Timed assessment and treatment coding, supervising-analyst and technician scrub, concurrent-billing checks, auth tracking, submission, posting, denials, and A/R.
How much do ABA therapy billing services cost?
The fee is a percentage of net collections. No setup fee on the monthly cycle. The written rate follows authorized hours and payer mix.
Are ABA units a visit level?
No. Codes such as 97151, 97153, and 97155 are timed in 15-minute increments from active service minutes — not an office-visit level.
Can 97153 and 97155 be billed at the same time?
Only when different qualified providers deliver distinct services that each meet the descriptor. One provider cannot bill both for the same minutes.
Do we have to change our EHR?
No. Billers work in the system you already use — see EHR integration. Compatibility is confirmed before you commit.
Where does Vector provide ABA billing?
The office is 15717 85th St, Jamaica, NY 11414-2620. Work is remote for ABA practices in all 50 states. See the specialties hub.
Is Vector Medical Billing HIPAA certified?
HHS does not certify billing companies. Vector signs a BAA, limits PHI to staff on the account, and follows HIPAA workflows. A HIPAA security risk analysis is a separate engagement when scoped.
