Substance Use Disorder Billing Services
Substance use disorder billing is intake, group, and individual codes for the program the payer enrolled. Vector Medical Billing, in Jamaica, New York, matches ASAM level and fee-schedule lines — and keeps OTP weekly bundles on the right payer — inside the EHR you already use.
What substance use disorder billing services include
Full-cycle RCM for outpatient SUD clinics, IOP/PHP programs, and OTPs — not a generic therapy visit with a different diagnosis.
The operating cycle matches medical billing services: eligibility and carve-out routing, charge capture from assessment and session notes, ICD-10 (typically F10–F19 when the plan requires a SUD primary) with the HCPCS or CPT the fee schedule lists, scrub, clearinghouse submission, payment posting, denial management, and A/R follow-up. The scrub watches enrolled program codes, HF or other required modifiers, level-of-care authorization, and OTP weekly-bundle frequency.
In scope: alcohol and/or drug assessment, individual and group counseling on the SUD schedule, ambulatory and residential withdrawal-management lines when the facility bills them professionally or under the program path you own, intensive outpatient and partial hospitalization, and opioid treatment program claims under Medicare G-bundles or the Medicaid code that state still uses. Broader community behavioral health program H-codes, CPT psychotherapy, and psychiatry E/M stay on those pages when the visit is not SUD-program work. Peer support is out of scope here. ABA therapy has its own page.
Substance use disorder is priced as a percentage of net collections with other behavioral-health work. The written rate follows outpatient counseling versus IOP/OTP mix — see pricing. Credentialing with each SUD network and OTP certification footprint, a retrospective coding audit, MIPS, and out-of-network / IDR stay separate engagements.
What sits on this SUD fee
- Intake, group, and individual lines matched to the enrolled program
- IOP, PHP, and OTP bundles scrubbed against auth and weekly rules
- HIPAA workflows, a signed BAA, and PHI limited to staff on the account
Where substance use disorder claims fail
Common substance use disorder billing issues include a psychotherapy code on a program that pays a different code, a level of care without remaining authorization, and opioid-treatment bundles billed twice in the same week.
Wrong code for the enrolled program
90837 billed into a Medicaid SUD schedule that only pays H0004. Group time billed as individual. Assessment H0001 repeated every week without a new assessment event the plan allows.
Use the intake, group, and individual codes for the program the payer enrolled.
Level of care without auth
H0015 IOP or H0035 PHP submitted without remaining authorized units. Ambulatory detox hours billed as outpatient counseling. ASAM level in the note does not match the code family on the claim.
Higher levels of care follow auth and documentation — not habit.
OTP bundle collisions
Two weekly G-bundles in the same seven-day window. Counseling unbundled on top of a weekly episode that already includes counseling. Medicare G2067 mixed with a Medicaid H0020 line for the same week without payer instruction.
One weekly episode code set per payer rules.
Intake, group, and individual for the enrolled program
The payer’s fee schedule picks the code. The name of the appointment does not.
On many Medicaid and MBHO SUD schedules, H0001 is alcohol and/or drug assessment (often one session for ASAM level 0.5 or 1 intake). Individual counseling is commonly H0004 in 15-minute units. Group counseling by a clinician is commonly H0005 per session. Commercial plans that still use CPT psychotherapy for SUD often require modifier HF. Do not invent HF on a Medicaid H-code claim that does not ask for it, and do not drop HF when the commercial fee schedule requires it.
Bill the code the program is enrolled to deliver. A licensed clinician’s psychotherapy under a medical-visit benefit belongs on the mental health path when that is how the payer pays. A certified SUD program’s H-code path is this page. Primary diagnosis is usually in the F10–F19 range when the state SUD schedule requires it.
Outpatient checklist
- Assessment vs individual vs group matches the note
- Units match the descriptor (15 min, session, or hour)
- Modifiers and taxonomy match that SUD enrollment
IOP, PHP, and withdrawal management
Level of care is a different claim family from a single counseling session.
IOP and PHP
H0015 is the common intensive outpatient alcohol and/or drug services code when the program meets the payer’s hour and day thresholds. H0035 is partial hospitalization when that is the enrolled level. Do not swap H0015 for an hourly cousin such as H2035 because the descriptors feel similar — units and documentation differ. Track prior auth by member and level of care before the claim outruns authorized days.
Detox and ASAM match
Ambulatory and residential withdrawal-management codes (for example H0014 ambulatory, H0010 / H0011 residential families) follow the ASAM withdrawal-management level documented — not a counseling code with a detox diagnosis. Facility room-and-board components often stay on a separate line or claim type. If your engagement is only the professional counseling piece, do not bill the facility per diem.
Opioid treatment programs
Weekly bundles on Medicare. State Medicaid may still use a different code.
Medicare weekly episodes
OTP Part B claims use weekly G-codes such as G2067 (methadone bundle) and G2068 (oral buprenorphine bundle). Bill one weekly episode per seven-day period when at least one service in the bundle was delivered. G2074 covers a weekly bundle without the drug when only non-drug components apply. Intake add-on G2076 is for new patients starting OTP treatment — not every reassessment.
Take-homes and counseling
Take-home supply add-ons (G2078 / G2079) have monthly frequency caps and must not overlap dates already covered by a medication weekly bundle. Extra counseling minutes may use G2080 when the week’s counseling exceeds what the bundle assumes. Do not unbundle ordinary counseling that the weekly code already includes.
Medicaid H0020 and POS
Some Medicaid programs still pay H0020 or another state OTP code; others have moved to Medicare-aligned G-bundles. Use the code set that state’s SUD billing guide lists for the date of service. OTP place of service is often 58; mobile medication units may use POS 15 when the plan requires it. Carve-out routing still applies — see behavioral health billing.
SUD code families this service works
Illustrative anchors — the enrolled fee schedule decides the payable line.
| Work type | Example | What to check | Common failure if skipped |
|---|---|---|---|
| Assessment / intake | H0001 | New assessment event | Weekly “intake” habit |
| Individual counseling | H0004 × units | Direct minutes | 90837 into H-schedule |
| Group counseling | H0005 | Session rules; clinician | Billed as individual |
| Commercial SUD therapy | 9083x + HF | Plan requires HF | Missing / extra HF |
| IOP | H0015 | Hours/days; auth | Wrong hourly cousin |
| PHP | H0035 | LOC auth | Billed as outpatient E/M |
| OTP Medicare | G2067 / G2068 | One week; ≥1 service | Duplicate weekly bundle |
| OTP Medicaid | H0020 or state G-set | That state’s guide | Mixed code sets one week |
| Diagnosis | F10–F19 primary | SUD schedule rules | Wrong primary DX |
This page does not invent a substance use disorder denial percentage. The free audit reads your program-code and OTP choices on the sample you send.
What a free audit line looks like
Illustrative composite only — not a client result and not a promised recovery.
| Field | Example entry |
|---|---|
| Documented service | ASAM Level 1 individual counseling, 45 minutes direct; member on Medicaid SUD schedule that lists H0004; primary diagnosis F11.20 |
| Billed codes | 90834-HF to the medical MCO payer ID |
| Issue found | Wrong code family for the enrolled program; possible carve-out routing miss |
| Recommended correction | Bill H0004 with three 15-minute units to the SUD / MBHO payer ID on the fee schedule; confirm HF is not required on that H-code |
| Related check | Taxonomy matches SUD program enrollment; auth not required for Level 1 on this plan |
| Why it matters | Medical-visit CPT into a program schedule denies as non-covered — caught in scrub or flagged in the free audit |

How Vector runs substance use disorder billing
Which program the payer enrolled first — then the same revenue cycle.
Assign SUD RCM
Coder and biller who already match H-codes, HF, and OTP weekly bundles.
Reconcile and code
Charge entry and coding from the assessment or session note — queries when LOC or units are unclear.
Post and report
835 posting, denial work, A/R aging, monthly RCM report by intake / group / individual / OTP.
Not inside the substance use disorder percentage
Related work stays separate so the quote matches the job.
Residential facility fees
Room-and-board and hospital facility charges stay on the facility claim. We bill the SUD program and professional lines your organization owns.
Credentialing / audit
Credentialing with SUD networks and OTP certification panels, and a retrospective coding audit, are separate when scoped.
Underpay / OON
Underpayment recovery and OON / IDR stay separate when the dispute path is not ordinary denial work.
How substance use disorder billing is priced
Percentage of dollars that land — not a flat fee per group that ignores OTP weekly edits.
Written rate
Substance use disorder is priced as a percentage of net collections because program codes, level-of-care auth, and OTP bundles take more scrub and follow-up time. No setup fee on the monthly cycle. Month to month. The written rate follows outpatient versus OTP mix and how much of the cycle you hand over.
See pricing or request a written rate after the free audit.
What we need to start
EHR access, SUD fee-schedule excerpts (H-codes and OTP codes), carve-out payer IDs, open A/R if you are leaving another biller, and a sample of program or OTP denials. Switching usually overlaps so claim submission does not stop. See EHR integration if access setup needs a separate quote.
Audit the SUD claims that still use a medical-visit code
Send monthly volume by intake, group, individual, IOP, and OTP, fee-schedule excerpts, EHR name, and a sample of non-covered denials. We reply with what the open A/R shows, whether the monthly fee fits, and a written rate — without inventing a clean-claim percentage.
Substance use disorder billing questions
Straight answers tied to the sections above.

Intake assessment, individual and group counseling on the enrolled schedule, IOP and PHP when authorized, OTP weekly bundles or state methadone codes, scrubbing, submission, posting, denials, and A/R.
How much do substance use disorder billing services cost?
The fee is a percentage of net collections. No setup fee on the monthly cycle. The written rate follows outpatient versus OTP mix.
Which codes are used for intake, group, and individual?
Many SUD schedules use H0001 for assessment, H0004 for individual counseling, and H0005 for group. Some commercial plans use CPT with modifier HF. Bill the set the payer enrolled.
How do OTPs bill Medicare?
Weekly G-bundles such as G2067 and G2068, with add-ons for intake and take-homes when criteria are met. Medicaid may still use H0020 or aligned G-codes — follow that state’s guide.
Do we have to change our EHR?
No. Billers work in the system you already use — see EHR integration. Compatibility is confirmed before you commit.
Where does Vector provide SUD billing?
