Behavioral Health Billing Services
Behavioral health billing is timed services — often paid under a program benefit rather than a medical-visit benefit. Vector Medical Billing, in Jamaica, New York, routes the carve-out, matches the HCPCS the state plan enrolled, and scrubs units from documented direct minutes inside the EHR you already use.
What behavioral health billing services include
Full-cycle RCM for community programs and clinic-based BH agencies — not a medical office visit with a different label.
The operating cycle matches medical billing services: eligibility that asks which entity owns the BH benefit, charge capture from timed notes, ICD-10 / HCPCS (and CPT when the fee schedule uses them), scrub, clearinghouse submission to the correct payer ID, payment posting, denial management, and A/R follow-up. The scrub watches carve-out routing, program code versus CPT psychotherapy, 15-minute unit math from direct member time, taxonomy and rendering enrollment, and prior auth units for higher levels of care.
In scope: community mental health and behavioral health clinic claims on the program fee schedule, HCPCS counseling and rehab lines the state or MBHO covers, intensive outpatient and partial-hospitalization program codes when documented, psychosocial rehab and skills training, case management when separately payable, and telehealth under that BH vendor’s rules. Dedicated timed CPT psychotherapy and psychiatry E/M with add-ons stay on those pages. Substance use disorder program billing has its own page. ABA therapy has its own page. Peer-support deep pages follow later in the hub queue.
Behavioral health is priced as a percentage of net collections. The written rate follows program mix and how many carve-out vendors you bill — see pricing. Credentialing with each MBHO and Medicaid program, a retrospective coding audit, MIPS, and out-of-network / IDR stay separate engagements.
What sits on this behavioral health fee
- Program benefit claims routed to the entity that actually pays BH
- Timed units scrubbed from documented direct minutes
- HIPAA workflows, a signed BAA, and PHI limited to staff on the account
Where behavioral health claims fail
Common behavioral health billing issues include sending a carved-out claim to the medical plan, using the wrong code family or unit count, and missing authorization or enrollment details.
Medical payer ID on a BH claim
A correctly coded H-code or therapy line sent to the medical MCO when the benefit is carved out to an MBHO. The denial looks like a network failure; the fix is rerouting, not rewriting the CPT.
Confirm which plan administers the behavioral health benefit before the first claim goes out.
Wrong code family or units
Office-visit E/M or 90837 used when the enrolled program pays H0004 in 15-minute units. IOP billed with the wrong per-diem or hourly cousin. Units counted from charting time, travel, or prep instead of direct member contact.
Timed services need the code the fee schedule enrolled — and the minutes that code allows.
Auth and enrollment gaps
PHP or IOP without remaining authorized units. Taxonomy or rendering NPI that does not match how the agency is enrolled with that state or MBHO. Multiple dates of service jammed onto one claim line when the plan requires one DOS per line.
Program payment follows the enrollment and auth the payer issued.
Timed services under a program benefit
The payer’s fee schedule decides the code set. Appointment length alone does not.
A medical-visit benefit usually pays CPT psychotherapy (90832–90837) or an office E/M under the medical plan. A program benefit often pays HCPCS lines the Medicaid agency or MBHO lists for community mental health — for example behavioral health counseling per 15 minutes (H0004), intensive outpatient alcohol and/or drug services (H0015), partial hospitalization (H0035), psychosocial rehab, skills training, or case management. Those are not visit levels. Billing 99214 or 90837 into a program that only recognizes the H-code is a clean claim to the wrong benefit.
Descriptors that sound alike are not interchangeable. H0015 (IOP) and hourly program codes such as H2035 are different units and different documentation expectations. Use the exact code on the fee schedule the agency is enrolled under, with the modifiers and place of service that schedule requires.
Program checklist
- Confirm the code is on that payer’s BH fee schedule
- Match units to the descriptor (15 min, hour, or per diem)
- Do not substitute a medical-visit CPT by habit
Carve-outs and who gets the claim
“Do you cover behavioral health?” is not the same question as “who pays the claim?”
Ask who administers BH
At intake — and again at plan-year changes — ask whether behavioral health is administered by the medical plan or carved out to a separate organization. If carved out, record the MBHO name, payer ID, claims address, and authorization phone. Put that payer ID on the account before the first claim. A denial that looks like “out of network” is often a routing error to the medical MCO.
Auth for higher levels of care
IOP, PHP, and many community rehab lines need prior authorization even when outpatient counseling does not. Auth units are not a guarantee of payment — eligibility, timely filing, and documentation still apply. Track remaining units by member and level of care so the claim does not outrun the auth.
Timed units and enrollment
Direct member time builds the unit. Enrollment decides whether that unit is payable.
Fifteen-minute math
For codes timed in 15-minute increments, many state manuals use the mid-point rule: one unit starts at eight minutes of direct contact, two units at 23 minutes, and so on. Count only time with the member or family in the billable activity. Prep, travel, and charting after the encounter do not add units. Document start and stop or total direct minutes in the note.
Taxonomy and rendering
Community mental health, rehab, and substance-use enrollments often require a specific taxonomy on the billing NPI and a rendering clinician enrolled for that service category. A code that is covered on the fee schedule still denies when the taxonomy or provider type does not match. That is a credentialing and enrollment problem — not a scrub rewrite.
Claim form and diagnosis
Most outpatient program professional claims stay on the CMS-1500 / 837P path. Facility or residential lines may need the UB-04. Many Medicaid BH programs expect a behavioral health diagnosis in the primary position. One service date per line when the plan forbids date ranges on a single line.
Behavioral health code families this service works
Illustrative anchors — the enrolled fee schedule decides which line is payable.
| Work type | Example | What to check | Common failure if skipped |
|---|---|---|---|
| Counseling (program) | H0004 × units | Direct minutes; mid-point | 90837 into an H-code plan |
| IOP | H0015 | Program hours; auth | Wrong hourly cousin code |
| PHP | H0035 | Level of care; auth units | Billed as outpatient E/M |
| Psychosocial rehab / skills | H2017 / H2014 families | Fee schedule; taxonomy | Wrong provider type |
| Case management | T1016 / T1017 / H0006 | Direct time; bundling rules | Units under 8 minutes |
| Carve-out routing | MBHO payer ID | Who administers BH | Sent to medical MCO |
| CPT psychotherapy | 90832–90837 | Medical-visit benefit | See mental health page |
| Psychiatry med + therapy | E/M + 90833 family | Two clocks | See psychiatry page |
This page does not invent a behavioral health denial percentage. The free audit reads your carve-out routing and program-code choices on the sample you send.
What a free audit line looks like
Illustrative composite only — not a client result and not a promised recovery.
| Field | Example entry |
|---|---|
| Documented service | Community MH counselor documents 45 minutes of direct individual counseling; member’s commercial plan carves BH to an MBHO; agency is enrolled on that MBHO’s H0004 fee schedule |
| Billed codes | 90834 to the medical plan payer ID |
| Issue found | Wrong code family for the program benefit; wrong payer ID for the carve-out |
| Recommended correction | Bill H0004 with three 15-minute units (38–52 minute band under mid-point rules the plan uses) to the MBHO payer ID; confirm auth if required |
| Related check | Taxonomy and rendering NPI match CMHC enrollment; primary diagnosis is a BH code the plan accepts |
| Why it matters | Medical-visit CPT to the wrong door denies as non-covered — caught in scrub or flagged in the free audit |

How Vector runs behavioral health billing
Who pays the BH benefit first — then the same revenue cycle.
Assign BH program RCM
Coder and biller who already route carve-outs and scrub HCPCS units.
Reconcile and code
Charge entry and coding from the timed note and fee schedule — queries when units or LOC are unclear.
Post and report
835 posting, denial work, A/R aging, monthly RCM report by program code and carve-out vendor.
Not inside the behavioral health percentage
Related work stays separate so the quote matches the job.
Inpatient facility fees
Hospital or residential facility charges stay on the facility claim. We bill the professional and program outpatient lines your agency owns.
Credentialing / audit
Credentialing with each MBHO and Medicaid program, and a retrospective coding audit of H-code units, are separate when scoped.
Underpay / OON
Underpayment recovery and OON / IDR stay separate when the dispute path is not ordinary denial work.
How behavioral health billing is priced
Percentage of dollars that land — not a flat fee per encounter that ignores carve-out reroutes.
Written rate
Behavioral health is priced as a percentage of net collections because program codes, carve-outs, and unit scrub take more follow-up time. No setup fee on the monthly cycle. Month to month. The written rate follows program mix and how much of the cycle you hand over.
See pricing or request a written rate after the free audit.
What we need to start
EHR access, list of MBHO and Medicaid program payer IDs, fee-schedule excerpts for the H-codes you bill, open A/R if you are leaving another biller, and a sample of carve-out or unit denials. Switching usually overlaps so claim submission does not stop. See EHR integration if access setup needs a separate quote.
Audit the program claims that still go to the medical plan
Send monthly encounter volume by H-code, carve-out vendor list, EHR name, and a sample of non-covered or wrong-payer denials. We reply with what the open A/R shows, whether the monthly fee fits, and a written rate — without inventing a clean-claim percentage.
Behavioral health billing questions
Straight answers tied to the sections above.
What does behavioral health billing include?
Carve-out routing, timed program HCPCS and related codes, unit scrub, prior auth for higher levels of care, submission, posting, denials, and A/R.
How much do behavioral health billing services cost?
The fee is a percentage of net collections. No setup fee on the monthly cycle. The written rate follows program mix and payer mix.
How is a program benefit different from a medical-visit benefit?
A medical-visit benefit usually pays CPT psychotherapy or office E/M. A program benefit often pays HCPCS H-codes or other state-plan codes under a BH fee schedule — not as a visit level.
What is a behavioral health carve-out?
Many medical plans route MH and SUD benefits to a separate MBHO. Eligibility on the medical card does not set the claim payer ID. Bill the carve-out entity with that vendor’s auth rules.
Do we have to change our EHR?
No. Billers work in the system you already use — see EHR integration. Compatibility is confirmed before you commit.
Where does Vector provide behavioral health billing?
The office is 15717 85th St, Jamaica, NY 11414-2620. Work is remote for BH programs in all 50 states. See the specialties hub.
Is Vector Medical Billing HIPAA certified?
HHS does not certify billing companies. Vector signs a BAA, limits PHI to staff on the account, and follows HIPAA workflows. A HIPAA security risk analysis is a separate engagement when scoped.
