Peer Support Billing Services
Peer support billing is the peer service the state plan covers — billed under that program, not as psychotherapy. Vector Medical Billing, in Jamaica, New York, matches H0038 or the state’s own peer code, the group modifier, and documented direct minutes inside the EHR you already use.
What peer support billing services include
Full-cycle RCM for certified peer specialists and the agencies that bill them — not a therapist claim with a different title.
The operating cycle matches medical billing services: eligibility and carve-out routing, charge capture from the peer note, the HCPCS the state plan or MBHO lists, scrub, clearinghouse submission, payment posting, denial management, and A/R follow-up. The scrub watches the peer code versus psychotherapy, 15-minute units from direct contact, group and family modifiers, daily caps, and whether the rendering peer is certified the way that state requires.
In scope: adult, youth, and family peer support on the Medicaid or MBHO schedule, individual and group peer encounters, and the mental-health versus substance-use modifier when that state splits them. Clinical psychotherapy, psychiatry, broader community H-code programs, and SUD counseling or OTP bundles stay on those pages. ABA is out of scope here.
Peer support is priced as a percentage of net collections with other behavioral-health work. The written rate follows encounter volume and how many state or MBHO fee schedules you bill — see pricing. Credentialing of the agency and peer panel, a retrospective coding audit, MIPS, and out-of-network / IDR stay separate engagements.
What sits on this peer support fee
- State-plan peer code, not a psychotherapy CPT
- Units and modifiers scrubbed against the note
- HIPAA workflows, a signed BAA, and PHI limited to staff on the account
Where peer support claims fail
Patterns state peer billing guides keep naming.
Psychotherapy code for peer work
A recovery conversation billed as 90834 or 90837 under a therapist NPI. Or H0038 sent to a medical plan that does not cover peer services at all.
Hub rule: the peer service the state plan covers — not psychotherapy.
Wrong units or missing group modifier
One unit for a 45-minute visit. Travel and note time counted as contact. A group billed as individual because HQ was left off. Family peer billed without HR or HS when that state requires them.
Fifteen-minute math and the modifier tell the payer who was in the room.
Uncertified peer or wrong code for the state
Services by a specialist who has not met that state’s certification or supervision rule. H0038 used in a state that pays peer work under a different HCPCS. Units past the daily cap without the authorization that cap requires.
The fee schedule and the certification rule decide payment.
The peer service the state plan covers
Hub language made operational: non-clinical recovery support, billed on the peer line that state lists.
Most Medicaid programs pay peer support as self-help/peer services under H0038, in 15-minute units. It is not an evaluation and management visit and it is not psychotherapy. Do not substitute 90832, 90834, or 90837 because the encounter lasted half an hour. A few states use a different code for the same work — Illinois has used H2014 for substance-use peer support, and other states have used H2015, H2016, H0046, or a group-only code. H2014 is skills training in some states and peer support in others. Use the code on that fee schedule for the date of service.
Coverage is a state-plan or MBHO benefit. A commercial medical plan that does not list peer services will not pay H0038 no matter how clean the note is. Route the claim to the behavioral health administrator when peer benefits are carved out — the same door as other program claims.
Program checklist
- Code is on that state’s peer fee schedule
- Not a psychotherapy CPT for the same minutes
- Payer ID is the plan that actually covers peer services
Modifiers, units, and who was present
The base code is often the same. The modifier and the clock are not optional.
Common modifiers
Many states add HQ when the peer service is a group. Family peer support may use HR when the member is present and HS when the member is not. Youth peer support may use HA. Some states split mental-health peer (HE) from substance-use peer (HF). These are examples from published billing guides, not a national modifier set. If the fee schedule is silent, do not add a modifier by habit.
Fifteen-minute units
Count direct time with the member (or the family, when that is the covered service). One unit commonly starts at eight minutes; two units at 23 minutes; three at 38. Do not bill under eight minutes. Do not count documentation, travel, or a supervision meeting as units. Daily caps — for example eight units unless authorized to exceed them — are state rules. Exceeding a medically unlikely edit or a state limit denies the extra units.
Certification, supervision, and enrollment
A correct H0038 still denies when the specialist is not allowed to render it.
State certification
CMS left training, certification, and supervision standards to each state. Some require a current peer credential before the first paid unit. Others allow a defined window after required training while certification is pending. Billing cannot invent a certificate number. If the specialist is outside that window, hold the claim.
Agency enrollment
Peer claims usually bill under the enrolled agency, with the rendering specialist identified the way that Medicaid program requires. The taxonomy and provider type must match a peer or rehabilitative enrollment — not a psychotherapy-only panel. Fixing that is credentialing, not a code change.
Part of a treatment plan
Many states pay peer support only as part of documented behavioral health or SUD treatment, tied to goals on the plan — not as a stand-alone social visit. The note should name the recovery activity and the direct minutes. A missing plan link is a documentation hold, not a reason to switch the line to psychotherapy.
Peer support code families this service works
Illustrative anchors — the state fee schedule decides the payable line.
| Work type | Example | What to check | Common failure if skipped |
|---|---|---|---|
| Individual peer | H0038 × units | Direct minutes | Billed as 90834 |
| Group peer | H0038-HQ | State requires HQ | Group billed as 1:1 |
| Family peer | HR / HS | Member present or not | Wrong family modifier |
| Youth peer | HA | Age and state rule | Adult code on a youth service |
| MH vs SUD peer | HE / HF | Only if that state splits them | Habit modifier |
| Other state code | H2014, H2015, H2016 | That fee schedule | H0038 where it is not peer |
| Unit cap | Daily / weekly limit | Auth to exceed | Extra units written off |
| Psychotherapy | 90832–90837 | Licensed clinician benefit | See mental health page |
This page does not invent a peer support denial percentage. The free audit reads your code, modifier, and unit choices on the sample you send.
What a free audit line looks like
Illustrative composite only — not a client result and not a promised recovery.
| Field | Example entry |
|---|---|
| Documented service | Certified peer specialist, 40 minutes of direct individual recovery support tied to the treatment plan; state fee schedule lists H0038 per 15 minutes; no group |
| Billed codes | 90834 under a therapist NPI to the medical plan |
| Issue found | Psychotherapy code and wrong payer for a peer service |
| Recommended correction | Bill H0038 × 3 units under the enrolled peer agency to the BH payer ID; do not add HQ |
| Related check | Specialist’s certification is current; daily unit cap not exceeded; note excludes travel and charting |
| Why it matters | A therapy code for peer work denies or recoups — caught in scrub or flagged in the free audit |

How Vector runs peer support billing
Which state plan covers the peer service first — then the same revenue cycle.
Assign peer RCM
Coder and biller who already keep H0038 off the psychotherapy path.
Reconcile and code
Charge entry and coding from the peer note — queries when minutes or group status are unclear.
Post and report
835 posting, denial work, A/R aging, monthly RCM report by individual versus group peer.
Not inside the peer support percentage
Related work stays separate so the quote matches the job.
Clinical therapy and meds
Psychotherapy and medication management stay on the mental health and psychiatry claims. We bill the peer line your agency owns.
Credentialing / audit
Credentialing of the agency and peer panel, and a retrospective coding audit of units and modifiers, are separate when scoped.
Underpay / OON
Underpayment recovery and OON / IDR stay separate when the dispute path is not ordinary denial work.
How peer support billing is priced
Percentage of dollars that land — not a flat fee per encounter that ignores unit recoupment.
Written rate
Peer support is priced as a percentage of net collections because state-plan codes, modifiers, and unit caps take more scrub and follow-up time. No setup fee on the monthly cycle. Month to month. The written rate follows volume and how much of the cycle you hand over.
See pricing or request a written rate after the free audit.
What we need to start
EHR access, the state or MBHO peer fee schedule, how peers are certified, open A/R if you are leaving another biller, and a sample of H0038 or modifier denials. Switching usually overlaps so claim submission does not stop. See EHR integration if access setup needs a separate quote.
Audit the peer visits that still bill as therapy
Send monthly peer volume, the state fee schedule you use, EHR name, and a sample of non-covered or unit denials. We reply with what the open A/R shows, whether the monthly fee fits, and a written rate — without inventing a clean-claim percentage.
Peer support billing questions
Straight answers tied to the sections above.
What does peer support billing include?
State-plan peer codes, 15-minute unit scrub, group and family modifiers when that state uses them, certification checks, submission, posting, denials, and A/R.
How much do peer support billing services cost?
The fee is a percentage of net collections. No setup fee on the monthly cycle. The written rate follows volume and payer mix.
Is peer support billed as psychotherapy?
No. Most Medicaid schedules use H0038, not 90832, 90834, or 90837. A few states use a different HCPCS. Bill the code on that program’s fee schedule.
How are H0038 units counted?
Usually one unit per 15 minutes of direct peer service, starting at eight minutes. Charting and travel do not count. Daily caps follow that state’s rules.
Do we have to change our EHR?
No. Billers work in the system you already use — see EHR integration. Compatibility is confirmed before you commit.
Where does Vector provide peer support billing?
The office is 15717 85th St, Jamaica, NY 11414-2620. Work is remote for peer programs in all 50 states. See the specialties hub.
Is Vector Medical Billing HIPAA certified?
HHS does not certify billing companies. Vector signs a BAA, limits PHI to staff on the account, and follows HIPAA workflows. A HIPAA security risk analysis is a separate engagement when scoped.
